- Description
- The Department of Veterans Affairs, Network Contracting Office (NCO) 17 intends to award a firm fixed price services contract to GE Healthcare, Waukesha, WI, to provide services for reinstalling the GE MacLab hemodynamic Patient Monitoring system in the cath lab located at the Audie L. Murphy VAMC, San Antonio, TX. NAICS 811219 applies. The statutory authority permitting other than full and open competition is 41 U.S.C.253 (c) (1), as implemented by Federal Acquisition Regulation (FAR) subpart 13,106-1, only one source is capable of providing the supplies or services required at the level of quality required because the supplies or services are unique or highly specialized. No other company is expected to respond to this requirement. The period of performance shall be from the date of award and will extend for 1 month hence. This notice of intent is not a request for competitive quotes. It is not a solicitation or request for offers. No solicitation package is available, but the text of the statement of work for the requirement is attached to this notice. However, any qualified vendor that determines it can meet the government's requirements may give written notification through e-mail to the Contracting Specialist, Jerry Hyndman, at gerald.hyndman@va.gov no later than 8 January 2016, 4:00 PM CST. No telephone responses will be accepted. Information received will be for the purpose of determining whether or not to conduct a competitive procurement. The determination to conduct a competitive procurement based on responses to this notice is solely within the discretion of the Contracting Officer. A determination not to compete is solely within the discretion of the Government. Any response to this notice must show clear and convincing evidence that competition would be advantageous to the Government. "Statement of Work A.GENERAL INFORMATION 1.Title of Project: Re-install the GE MacLab Hemodynamic Monitoring system in Cath Lab 2 at Audie L. Murphy VAMC, San Antonio, TX, 78229-4404. 2.Scope of Work: Background: Cath Lab 2 (C622) went under construction in October 2015 to renovate the room for a new cath lab x-ray system. At that time, the GE MacLab Hemodynamic Patient Monitoring system used in that Cath Lab was removed and stored temporarily until the room was completed. Construction is scheduled to be complete 1/14/16, and the new x-ray equipment install will take place from 1/19/16 to 2/12/16. This GE MacLab system will need to be re-installed during the timeframe of the x-ray equipment install, so that it is up and running when the room is ready for patient use. Scope: The contractor shall provide all labor, materials, tools, specialty tools, and travel to re-install, set-up and configure the GE MacLab Hemodynamic Patient Monitoring system in Cath Lab 2 (C622). This system consists of a workstation in the control room, a Patient Data Module (PDM) in the procedure room, and a monitor on the x-ray monitor boom. Once the system is re-installed and configured, the contractor will verify proper operation and conduct a safety inspection to ensure the system is safe for patient use. The contractor shall also provide the following cables for the MacLab system: Cable PN 418335-008, Video Cable PN 2003442-003, and Power Cable PN 403689-010. These cables will be provided during the install of the x-ray system (1/19/16 - 2/12/16), so that the x-ray system manufacturer can run them through their equipment conduits. GE Healthcare is the manufacturer of the MacLab Hemodynamic Patient Monitoring System. The contractor field service engineers must be thoroughly trained and equipped to service any and all situations with this equipment. Information Systems Officer, Information Protection: The contractor will not have access to VA Desktop computers nor will they have access to online resources belonging to the government while conducting services. If removal of equipment from the VA is required, any memory storage devices, such as hard drives, solid state drives and non-volatile memory units will remain in VA control and will not be removed from VA custody. Privacy Officer: The contractor will not have access to protected Patient Health Information (PHI) nor will they have the capability of accessing patient information during the services provided to the VA and if removal of equipment from the VA is required, any memory storage devices, such as hard drives, solid state drives and non-volatile memory units will remain in VA control and will not be removed from VA custody. All research data available for Contractor analyses is de-identified. Records Manager: There will be no federal records created, maintained, used or disposition with this contract; Regardless of format [paper, electronic, etc.] or mode of transmission [e-mail, fax, etc.]. Safety Requirements: The Contractor is required to follow all applicable OSHA regulations. Contracting Officer shall notify the Contractor of any safety issues and the action necessary to correct these issues. Such notice, when served to the Contractor or his representative at the work site shall be deemed sufficient for the corrective actions to be taken. If the Contractor fails or refuses to comply promptly, the Contracting Officer may issue an order stopping all or part of the work, and hold the Contractor in default. 3.Performance Period: The work will be performed during normal business hours. The contractor must initiate work no later than 30 days after official notice to proceed is given by the Contracting Officer and the work will be completed no later than 45 days after that. 4.Place of Performance: The contract will be performed at the Audie L Murphy Hospital, South Texas Veterans Health Care System, located at 7400 Merton Minter Blvd., San Antonio, Texas 78229-4404. B.CONTRACT AWARD MEETING The contractor shall not commence performance on the tasks in this SOW until the CO has conducted a kick off meeting or has advised the contractor that a kick off meeting is waived. C.GENERAL REQUIREMENTS The intent of this Statement of Work is to provide a broad overview of the requirements, not to list all the requirements. Partial listing of the requirements in this Statement of Work does not reduce or eliminate the obligation of the Contractor to meet all manufacturers' and code requirements. D.CHANGES TO STATEMENT OF WORK Any changes to this SOW shall be authorized and approved only through written correspondence from the CO. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor. E.TRAVEL Price includes the cost of travel and labor for technicians. Any expenses for travel, lodging, per diem, and labor of contractor employees are the sole responsibility of the contractor. F.CONTRACTOR EXPERIENCE REQUIREMENTS - KEY PERSONNEL Contractor must be authorized to install, set-up, and configure the GE MacLab Hemodynamic Patient Monitoring System. All personnel must provide proof that they are licensed to work in the state of Texas and in their field of expertise. All personnel must also provide proof that they are allowed/certified to provide service on the GE MacLab Hemodynamic Patient Monitoring Sytstem. Credentials shall be submitted to the Contracting Officer's Representative (COR) upon request. "The COR duties are not delegable and all responsibilities shall revert back to the Contracting Officer, should the COR be unable to discharge their responsibilities. "Inspection and Acceptance shall be by the COR assigned to this contract. "The COR shall request security badges,(as applicable) through his/her immediate supervisor, for all contractor and subcontractor employees needing physical access to Government facilities; ensure that contractor employees complete pre-employment security awareness training and refresher training when required; ensure that contractor employees sign the Certificate of Confidentiality and Non-disclosure and a copy is provided to the Contracting Officer; coordinate the collection of necessary information from contractors to meet background investigation requirements for contractor personnel; and initiate the appropriate steps for contractor employees when the contract is complete or the contractor employee leaves for any reason. Under no circumstances will the contractor assume that the contract has been extended or otherwise renewed unless informed by the Contracting Officer.
