Control # 15-318 Limited Sources Justification or Sole Source Justification for Acquisitions under the Simplified Acquisition Threshold (Simplified Acquisition Threshold is $150K) 1. Contracting Activity: This requirement is for Evans Army Community Hospital. The contracting activity is Western Regional Contracting Office. 2. Description of Action: Requirement: 0 New ~ Repeat Pricing: ~ Firm-Fixed Price D Time & Materials 0 Cost Funds: ~ OMA 0 Other Funds: Total Dollar Value: $100,557.42 Name of Intended Contractor: Roche Diagnostics Street Address: 9115 Hague Road City, State, Zip: Indianapolis, IN 46250-0457 Phone: 800-428-5074 3. Description of Supplies/Services: (Describe below the services or supplies, including make & model number, to be acquired.) The Point of Care INR testing meters, supplies, and software connectivity are critical Blood Coagulation testing required for anticoagulation patients. The equipment currently in place and that are used for patient care consist of CoaguChek XS Professional Meter Care Kit, CoaguChek XS Plus Meter Care Kit, CoaguChek XS Pro Meter Care Kit, CoaguChek XS Plus Base Unit, and CoaguChek XS Plus Rechargeable Battery Pack. Consumables needed are CoaguChek XS PT Test Strips, CoaguCheck XS Plus Quality Control, and CoaguChek XS Lancets 200ct. The Software Connectivity are MAS RALS Plus CoaguChek Annual Licenses, MAS RALS Web Based Remote Access, and Connected Meter Quantity. 4. Authority Cited: FAR 13.106-1(b)(1)(i): Only one source reasonably available (e.g., urgency, exclusive licensing agreement, brand name or industrial mobilization); OR 5. Reason for Authority Cited: (State why this is the only source that can provide the required supplies or services.) Current Roche Diagnostics INR Point of Care testing that is in place at the Internal Medicine Clinic in Evans Army Community Hospital. Brief Internet Survey of Pricing for similar services. The current testing provided through the expiring contract is working well. Based on patient load and projected patient load, we still require the same number of test strips and quality control kits. MAS RALS connectivity is a requirement for lab accreditation and must also continue. 6. Actions to Increase Competition: (State what actions will be taken to increase competition before subsequent acquisition of the supplies or services is required. If NIA, state why this is not applicable- are they the only company that provides this product?) Actions may be solicited as Brand Name only due to the requirement for expanded MAS/RALS connectivity and testing options. 7. Market Survey: (Describe the extent of the market survey conducted to identify all qualified sources and the results thereof. "Market Survey" is defined as those attempts you made to ascertain whether other qualified sources exist. The survey of the market place may consist of written, telephonic, or world wide web inquiries. Did you go on line? If so, what websites did you use? GSA.gov, Salary.com, Indeed.com, etc.) The market research was completed in accordance with applicable FAR 10.002b(2) procedures, with the following results: This is a follow-on requirement for point of care INR testing meters, supplies, and software connectivity; The associated NAICS code for this purchase is unknown. A Dynamic Small Business Search was not conducted; A review of the Federal Supply Schedule Contracts was not performed; Market research revealed that Cardinal Health may be able to meet this requirement, however, MAS/RALS connectivity is purchased through Roche diagnostics. 8.
