- This is a sources sought for the DC VAMC located at 50 Irving Street NW, Washington, DC 20422. This notice is to determine the availability of Small Businesses as potential sources under NAICS 811213 communication equipment repair and maintenance. All interested should submit a capability statement and indicate social economic status. Include the DUNS number of your firm. State whether your firm is registered with the System for Award Management (SAM) at www.sam.gov and/or the VetBiz Registry at www.vetbiz.gov and indicate if your firm has a GSA Schedule. Please Note: any future solicitation could only be awarded to a contractor who is registered in SAM. To receive award based on VOSB or SDVOSB status you must be registered in the VetBiz Registry. Please respond via email to vanessa.jacobs@va.gov by 12:00pm EST January 27, 2016. Provide only the requested information above. STATEMENT OF WORK SCOPE OF WORK: The Contractor shall provide all labor, personnel, equipment, tools, materials, supervision and other items and services necessary to service and maintain the televisions, television arms, and patient TV and RF (signal) at the Washington DC Veterans Affairs Medical Center, 50 Irving Street NW, Washington, DC 20422. Two preventive maintenance visits for head end and distribution equipment maintenance shall be provided and performed during regular service hours. The preventive maintenance shall include the following: "Complete RF signal check from each modulator on the rack "Inspection of video equipment and receivers to be sure that the audio and video levels are sufficient and balanced "Complete RF Check of all wing amplifiers "System will be adjusted and balanced as necessary The Contractor shall provide six major outage emergency calls not to exceed eight hours. Contract also includes open PO for parts in the amount of $8,000. Scope includes televisions throughout the facility, including but not limited to waiting rooms, patient rooms, conference rooms, offices, etc. The inpatient television system currently used by the Medical Center is GetWell. 1. EQUIPMENT: The contractor shall provide all tools and equipment necessary to properly and safely perform the services set forth within this statement of work. This shall include, but is not limited, to all Personal Protective Equipment (PPE) which may be required for the work to be completed in a safe manner. 2. AREAS TO BE SERVICED: (one or all as necessary and appropriate to this contract) Washington DC VA Medical Center 50 Irving Street NW Washington, DC 20422 3. TASK FREQUENCY AND INSTRUCTIONS: The Contractor shall provide three weekly visits and remain onsite for no less than 16 hours per week. The Contractor shall perform services between 7am and 6pm. The Contractor shall provide an emergency call response time of six (6) hours. Any additional time shall be scheduled in advance with the Washington DC VA Medical Center's Biomedical Engineering Section. Service reports for any and all work done are required to be submitted to the Biomedical Engineering Section within three business days of completion of the work. 4. SPECIAL WORK REQUIREMENTS: None. 5. PROPERTY DAMAGE: The contractor shall take all necessary precautions to prevent damage to any government property. The contractor shall report any damages immediately and shall be assessed current replacement costs for property damaged by the contractor, unless corrective action is taken. Any damaged material (i.e., trees, shrubs, lawn/turf, cubs, gutters, sidewalks, etc.) will be replaced in a timely manner or corrected by the contractor with like materials, at no extra cost to the government, upon approval of the Contracting Officer. 6. ATTACHMENTS: None. 7. IDENTIFICATION, PARKING, SMOKING, CELLULAR PHONE USE AND VA REGULATIONS: The contractor's employees shall wear visible identification at all times while on the premises of the VA property. It is the responsibility of the contractor to park in the appropriate designated parking areas. Information on parking is available from the VA Police-Security Service. The VA will not invalidate or make reimbursement for parking violations of the contractor under any con-ditions. Smoking is prohibited inside any buildings at the VA. Cellular phones and two way radios are not to be used within six feet of any medical equipment. Possession of weapons is prohibited. Enclosed containers, including tool kits, shall be subject to search. Violations of VA regulations may result in a citation answerable in the United States (Federal) District Court, not a local district state, or municipal court. 8. COMPLIANCE WITH OSHA BLOODBORNE PATHOGENS STANDARD: The contractor shall comply with the Federal OSHA Bloodborne Pathogens Standard. The contractor shall: A. Have methods by which all employees are educated as to risks associated with bloodborne pathogens. B. Have policies and procedures which reduce the risk of employee exposure to bloodborne pathogens. C. Have mechanisms for employee counseling and treatment following exposure to bloodborne pathogens. D. Provide appropriate personal protective equipment/clothing such as gloves, gowns, masks, protective eyewear, mouthpieces for the employee during performance of the contract.
