General Information THIS IS SYNOPSIS for the purchase titled for Behavioral Health Optimization Program Provider for 374th Medical Group, Yokota Air Base, Japan. The purpose of this action is to synopsis to purchase the subject services. The Government reserves the right to cancel this solicitation, either before or after the closing date. B. Scope of Work The requirements were 1) to function in a specialty mental health clinic or in a patient centered medical home location as a behavioral health consultant and/or behavioral health care facilitator, 2) to attend and participate in meetings, professional staff conferences and other appropriate professional activities, 3) to provide clinical/counseling services, 4) to prepare all documentation to meet or exceed stablished standard of medical treatment facility, in accordance with the attached PWS. C. Planned Evaluation Criteria Lowest Price Technically Acceptable with Acceptable Past Performance. This is a competitive best value source selection in which Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, technical, price, and past performance considered. The Government intends to make award to the offeror who represents the best value to the Government. The Government intends to award without discussion with respective offerors. The Government, however, reserves the right to conduct discussions if deemed in its best interest.
