Description

Amend SCOPE of WORK And provide picture of broken damper see attachment. Close date remains the same. Scope of Work REPLACEMENT OF ONE FAILED DAMPERS IN BUILDING 1 1.The Contractor shall provide all materials, labor, and supplies necessary to replace two failed dampers located in building 1. Building 1 is located at the Bay Pines VA Healthcare Center, 10000 Bay Pines Blvd., Bay Pines, Florida 33744. 2.During TJC inspection Damper 3-FD-009 failed due to rust and springs broken on damper. 3.Dampers cannot be repaired due to condition and replacement is imperative to functionality of the system. Work shall include re-inspect of dampers after replacement. 4.Vendor will perform all operations in a prudent, conscientious, safe and professional manner and will comply with all safety requirements set forth in applicable State, Federal and Local laws and regulations. 5.Vendor will provide the necessary personnel, all materials and equipment to successfully install and commission fire suppression system. Coordination with Engineering and SEM will be required. 6.Work to be completed within 30 days. Hours 7:30 AM - 4:30 PM
