Description

1. The Department of Veterans Affairs (VA), Veterans Integrated Service Network 15 (VISN 15), encompassing 7 VA Health Care Systems (8 VA Medical Centers) in - - Marion, Illinois - serving Southern Illinois, Southern Indiana and Western Kentucky - Poplar Bluff, Missouri - serving Southeastern and South Central Missouri and Northwest Arkansas - St. Louis, Missouri - serving St. Louis Metropolitan Area and Eastern Missouri - Columbia, Missouri - serving Central and Northern Missouri - Kansas City, Missouri - serving Kansas City Metropolitan Area, Northwestern and Western Missouri, and portions of Southwest Kansas - Eastern Kansas Health Care System - serving Eastern, Northeastern and Central Kansas - Wichita, Kansas - serving Southern, Central, Northern and Western Kansas 2. Network Contracting Office 15 (NCO 15)intends to issue an Open and Continuous Solicitation for Community Nursing Home Services for an open period of tow (2) years to accept offers for nursing home services for the geographic areas listed above. The anticipated date of issue is on or about 1 April, 2016. The estimated closing date will be 2 calendar years from the issue date. 3. Previous Open and Continuous Solicitations have been issued for the same type of services. The current solicitation, VA255-14-R-0201, is expected to expire on 29 February, 2016, unless further extended. 4. NCO 15 is seeking responses by potentially interested offerors for provision of nursing home care services in the areas indicated in the attached maps of the VISN 15 service area (States and Counties). Further - NCO 15 is requesting feedback from potentially interested nursing home facilities (potential offerors) to indicate the nursing home facility resources in or near the areas specified in paragraph 4. NCO 15 is requesting feedback from potentially interested potential offerors in areas not specifically listed, but within the VISN 15 coverage area identified in specified in paragraph 1. NCO 15 requests identification of: - Nursing Home Facility Name- City, County, State - Indicate if currently, or previously provided services to a VA Medical Center - Special Care Capabilities - e.g. Trach/Vent Weaning, Rehabilitation Capabilities, Behavioral, Alzheimer/Dementia Care, Secure Facility, etc. - Capacities - Total and Typical Occupancy - Socio-Economic Status of Offeror - e.g. 8(a) Certified, Service-Disabled Veteran-Owned Business, Veteran-Owned Business, Small Disadvantaged Business, HUBZone Business, Woman or Women-Owned Business, Small or Large Business. - Current Medicaid Reimbursement Rate, date of last Medicaid rate revision, frequency of revisions by your State Medicaid Authority/rate-setter. - Private-Pay room and board rates, the previous rates and approximate date of last adjustment, frequency of adjustments. - Indicate if a social worker is on staff, other staffing capabilities - Current CMS/Medicaid Survey (rating, last survey date, re-inspection information, results) (summary only, not full documents) If all information above is not provided, please provide, as a minimum: - Nursing Home Facility Name- City, County, State - Indicate if currently, or previously provided services to a VA Medical Center - Special Care Capabilities - e.g. Trach/Vent Weaning, Rehabilitation Capabilities, Behavioral, Alzheimer/Dementia Care, Secure Facility, etc. - Capacities - Total and Typical Occupancy - Socio-Economic Status of Offeror - e.g. 8(a) Certified, Service-Disabled Veteran-Owned Business, Veteran-Owned Business, Small Disadvantaged Business, HUBZone Business, Woman or Women-Owned Business, Small or Large Business. - Current Medicaid Reimbursement Rate, date of last Medicaid rate revision, frequency of revisions by your State Medicaid Authority/rate-setter.
