- THIS IS A SPECIAL NOTICE OF THE VISN 7 CONTRACTING NETWORK'S INTENT TO SOLE SOURCE. NO HARD COPY OF THE RFQ IS AVAILABLE FOR DISTRIBUTION. This notice is being published in accordance with Federal Acquisition Regulation (FAR) Part 5.101(a)(1) dissemination of information on proposed contract actions. The Government anticipates entering into sole source procurement with Prism Medical, Inc., to provide Preventive Maintenance on 129 ceiling lifts installed by Prism Medical at the Tuscaloosa VA Medical Center. The ceiling lifts are currently under warranty and if they are not serviced by Prism Medical or its authorized service agents the warranty shall be void. SEE ATTACHED SOW This announcement is for services for which the Government intends to solicit and negotiate with only one source under FAR 6.302-1. Other persons may identify their interest and capability to respond to this requirement by March 7, 2016 at 4:00 PM CST. This Notice of Intent is not a request for competitive proposals. However, all interest received on or before the date and time indicated above will be considered by the Government. A determination by the Government not to open the requirement to competition based upon responses to this notice is solely within the discretion of the Government. Information received as a result of the notice of intent will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. No solicitation document is available. STATEMENT OF WORK A. GENERAL: 1. The Full Service Agreement shall provide all parts (excluding soft goods such as slings), on site labor, preventative maintenance, visual inspections with load testing as outlined below, and be certified to work on the Waverly Glen Ceiling Lifts. Service calls will require product screening by the facility, following determination of the need for the service technician should be contacted through customer service. All repairs will be performed by a local service representative. Contractor shall provide loaner lifts and will be available on site to decrease down time and typical response time is within 48 hours. 2. Description: As a result of the service contract, veterans at Tuscaloosa VA Medical Center will receive care using the safest lifting equipment possible. Additionally, this contract will ensure that staff is performing their work assignments in a safe and injury free environment. To ensure that this has occurred to the satisfaction of the facility, the inspector will be required to provide a checklist outlining the actual service completed. This checklist will be provided to the Safe Patient Handling Coordinator for the facility as indicated below in Section B (Specifications). 3. Location of Services: The work will be performed in the buildings, specific floors and rooms where ceiling and manual lifts are located. These include Buildings 137, 135, 61, 146 and 38. B. SPECIFICATIONS: The vendor shall provide the following services during the general inspection and preventative maintenance for track lift products: Visual inspection 2X yearly (Ceiling Lift & Track products): - The lift lifting tape shows no signs of fraying or breaking along its entire length - The stitching on the lift lifting tape where it connects to the carry bar shows no signs of fraying, or breaking. - The sling (s) that will be used shows no signs of unusual wear and tear. The straps of the sling that connect to the carry bar of the lift show no signs of fraying or breaking. Refer to specific sling instructions. - The airline tube that connects the hand control to the lift is not kinked, twisted, knotted, cut or damaged. - All the functions on the hand control work correctly (e.g. UP/DOWN/LEFT/RIGHT, etc.) - The brackets that hold the track in place on the ceiling are secure and do not move or appear loose. - There are no cuts, dents or sharp edges on the carry bar that may damage the straps of the sling. - Ensure the lift makes no unusual sounds when the carry bar is moved UP/DOWN or the lift is moved LEFT/RIGHT. - Ensure that there are end stops installed at each end of the track. - Ensure the lift moves freely along the entire length of the track. - Certify and record of lift inspection for inspector records Preventative Maintenance 2X yearly (Ceiling Lift & Track products): - Complete all steps outlined above under Visual Inspection - Record then reset the PM lifts counter - Inspect complete lift operation - Inspect all power connections and record all power volts - Check and load test Batteries - Lubricate moving parts as needed - Clean all rails and trolleys within rails - Check strap limit switches - Certify and record of lift inspection for inspector records Load Testing to EN10535 standards 1X yearly (Ceiling Lift & Track products): - Tracks will be load tested to 1.25 times the safe working load; each track attachment will be point load tested. The deflection of the track will be measured and determined if it is acceptable to (EN10535) standards. - Lifts will be load tested per manufactures specifications - Inspect above ceiling attachments - Certify and record of lift inspection for inspector records Weight Scale Testing and Calibration 1X yearly (Ceiling Lift & Track products): - Weight scales will be calibrated if the testing proves not to be accurate and then tested again once calibration is complete - Certify and record of weight scale testing The vendor shall provide the following services during the general inspection and preventative maintenance for floor lift products: Visual inspection 2X yearly (Floor based products): - The sling (s) that will be used shows no signs of unusual wear and tear. The straps of the sling that connect to the carry bar of the lift show no signs of fraying or breaking. Refer to specific sling instructions. - The airline tube that connects the hand control to the lift is not kinked, twisted, knotted, cut or damaged. - All the functions on the hand control work correctly (e.g. UP/DOWN/.LEFT/RIGHT, etc.) - There are no cuts, dents or sharp edges on the carry bar that may damage the straps of the sling. - Ensure the lift makes no unusual sounds when the carry bar is moved UP/DOWN - Inspection and cleaning of the casters and brakes of floor based products - Inspection of the structural integrity of the mast, lift arm and base - Inspection of the actuator for structural integrity, wear and looseness at lift arm - Certify and record of lift inspection for inspector records Preventative Maintenance 2X yearly (Floor based products): - Complete all steps outlined above under Visual Inspection in addition to: - Inspect complete lift operation - Check and load test Batteries - Lubricate moving parts as needed - Load test to maximum load of lift per manufacturer's specifications - Certify and provide record of lift inspection for inspector records C. HOURS OF WORK: The Typical response time for the service technical will be 48 hours. There will be loaner lifts on site to decrease down time and to ensure continuity of care.
