- To purchase 43 annual licensing software and subscription support services for the Phoenix Indian Medical Center. Combined Synopsis/Solicitation IAW 12.603 (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.603 - Streamlined solicitation for commercial items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation proposals are being requested and a written solicitation will not be issued. (ii) Request for Quotation number IHS1279084. (iii) This Request for Quotation solicitation document and incorporates provisions and clauses are those in effect through the most current Federal Acquisition Circular. (iv) The North American Industry Classification System (NAICS) code is 541519 and the business size standard is $27.5 million. (v) All quotes shall be in accordance with FAR 52.247-34, FOB Destination (vi) The Phoenix Area Indian Health Service, for the Phoenix Indian Medical Center, is requesting quotes for the Purchase of 43 annual licensing software and subscription support services. (vii) Place of Performance is at the Phoenix Indian Medical Center, 4212 North 16th Street, Phoenix, Arizona 85016, period of performance is 10 days after award and this location is where delivery and acceptance of requested equipment and installation services. (viii) The following FAR Clause provisions apply to this solicitation; FAR 52.212-1 - Instructions to Offerors - Commercial Items (Oct 2015) (ix) The following FAR Clause provisions apply to this solicitation; 52.212-2 - Evaluation - Commercial Items (Oct 2014). The Government intends to award a firm-fixed price purchase order resulting from this solicitation. Award will be made to the best value to the Government. (x) The following FAR Clause apply to this solicitation; FAR 52.212-3 - Offerors Representation and Certifications - Commercial Items (Nov 2015). (xi) The following FAR Clause apply to this solicitation; FAR 52.212-4 - Contract Terms and Conditions - Commercial Items (May 2015). (xii) The following FAR Clause apply to this solicitation; FAR 52.212-5 - Contract Terms and Conditions Required To Implement Statues Or Executive Orders (Feb 2016). (xiii) The Contractor shall comply with the following Federal Acquisition Regulation (FAR) clauses, which are incorporated in this contract by reference, to implement provisions of law or Executive Orders applicable to acquisitions of commercial items: a. The Contractor shall comply with the following with the listed FAR and HHSAR Clauses. (xiv) Not applicable. (xv) All responsible sources may submit a quotation by February 26, 2016 at 10:00 am, MST, which if timely received, shall be registered with the System for Award Management (SAM) and shall provide the company Tax Information Number (TIN) with their quote.
