SOURCES SOUGHT NOTICE NOTE: THIS IS A SOURCES SOUGHT NOTICE REQUESTING INFORMATION. THIS IS NOT A SOLICITATION. NO PROPOSALS ARE BEING REQUESTED OR ACCEPTED AT THIS TIME. The Bay Pines VA Medical Center is conducting market research to determine the availability of qualified, technically capable vendors to furnish all parts, labor and transportation to provide preventive maintenance, software upgrades, and repair of the subject Siemens brand Biograph 64 PET/CT and Syngo Multi-Modality Workstation equipment located 10,000 Bay Pines Blvd, Bay Pines, FL 33744 in accordance with the below Statement of Work (SOW). The resultant contact shall be for a five (5) month base year (4/24/2016 - 9/30/2016) and one (1) option year. STATEMENT OF WORK (SOW) I.The contractor shall provide all repair parts (excluding glassware), transportation and service for maintenance on the Siemens Medical Solutions PET/CT Scanner EE# 98640 SN:1062, Model Biograph MCT-S 64, Workstation EE#98641, SN:4933 Model: Fujitsu Celsius M720 and Workstation EE#98644 SN:1409 Model: Fujitsu Celsius M720 located at the Bay Pines VA Healthcare System, Florida. The Scanner is located on the second floor, building 100, in room # 2C119D-100BP. The system maintenance covers all portions of the system, including the system control computer, monitors, cooling system and the Model# PET-20C27/2 SN# K003 (BIV-R SOURCE KIT FOR SIEMENS BIOGRAPH) phantom. Additionally, all software updates and upgrades which have been validated by the OEM shall be included in this maintenance plan. II.First call discount applies to this contract. Biomedical Engineering will be notified first to assess the problem. Ph#727-398-6661 Ext.15601. III.Hours of coverage will be from 8:00 AM through 10:00 PM, Monday through Friday, excluding Federal Holidays (New Year's Day, Martin Luther King Day, President's Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day). All work done outside the normal coverage time should be first approved through the Biomedical Engineering Section, VA Bay Pines. Preventive maintenance should be negotiated, during normal hours, at least one month in advance with the Nuclear Medicine section of Radiology Service at (727)398-6661 Ext. #14424. Preventive maintenance will be done at the manufacturer's recommended frequency. IV.Hardware and software upgrades will be scheduled and performed during normal working hours, at no additional charge to the VA. These upgrades are those required for the correct function of the system; not those purchased to enhance function. V.The contractor will provide all hardware and software (OEM parts only) necessary for unscheduled repairs. The contractor's field service engineer will respond by phone within 30 minutes, and respond on site within 4 hours to correct the problem. Uptime requirements, excluding scheduled preventive maintenance, will be 97% of the normal working hours and working days during the term of the contract. All broken parts removed to repair the system, will become the property of the contractor, with the exception of material containing electronic protected healthcare information. VI.The contractor must have trained personnel within a 100 mile radius of Bay Pines, Florida, with an adequate spare parts inventory in some area office and proof of manufacturer's or equivalent maintenance training of field service personnel. Remote diagnostic capability is at the expense of the contractor. The contractor must have the ability to remotely access the system for diagnosis and repair, thus improving system uptime. VII.The contractor will provide all service manuals, and will not have access to manuals purchased by the VA. VIII.Each scheduled and unscheduled visit will result in the production of a written report, by the contractor's representative, in a format negotiated between the VA and the contractor. The contractor will always contact the Biomedical Engineering Section, (727) 398-6661 extension 15601 (after hours, extension 17777) before and after arrival at the site.
