The Department of Veterans Affairs, Northport VA Medical Center is currently conducting a market survey for qualified firms capable of providing the requested Services The vendor will be responsible for, but not limited to providing the following services for brand name or equal equipment for: Leasing of Stryker Surgical Saw System Introduction The requirement entails the leasing of a new corded power surgical saw system for the operating room surgeons to use for Shoulder, Hip and knee procedures. The corded surgical power saw system would be used in Northport VAMC, 79 Middleville Road, Northport NY 11768. Objective The primary objective for procurement of the power surgical saw system is to provide more reliable care to the patient and staff by replacing current outdated equipment that is being used. The saw system will be available for use by Podiatry and Orthopedic surgeons and used for approximately 200 cases annually. The surgical saw system for leasing will include the following items: A system 7 dual trigger rotary A system 7 reciprocating saw A system 7 Sagittal saw Pin Collet An AO small attachment A Hudson/modified trinkle attachment A ¼" chuck w/key A universal charger A system 6 aseptattery kit The North American Industry Classification Systems (NAICS) is 339112. Responses to this notice must be submitted in writing via email and must be received not later than March 4, 2016 1:00 PM EST. No telephone inquiries will be accepted. The purpose of this notice is to gain knowledge of potential qualified sources and their size classification//socioeconomic status (service-disabled veteran owned small business, veteran owned small business, HUB Zone, 8(a), small business or large business), relative to NAICS 339112. Responses to this notice will be used by the Government to make appropriate acquisition decisions. Responses to this notice should include company/individual name, a service capability statement, examples of similar facilities for which you have provided services to, proof of applicable certification. DUNS number, address, point of contact and social-economic category (ex: SDVOSB, VOSB, 8(a), HUBZone, WOSB, EDWOSB, Small Business).
