Scope of Work. Pricing Schedule. Provisions and Clauses. Salient Characteristics. Combined Solicitation / Synopsis Notice. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a Request for Proposal (RFQ) and the reference number is IHSWRSUOptometry. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-83 and the Health and Human Services Acquisition Regulation (HHSAR). The Offeror(s) must quote on an all or none basis. Written quotes are required (oral offers will not be accepted). Submission of quotes must be in accordance with FAR 52.212-1, Instructions to Offerors, Commercial Item and FAR 52.212-2 Evaluation. The basis for evaluation shall on a lowest priced technically acceptable. Acquisition Procedures in accordance with FAR 13.106- 2/3.
