Attachment 7, Past Performance Information Form, 1 page Attachment 6, Past Performance Questionnaire Form, 2 Pages Attachment 5, Wage Determination, 10 Pages Attachment 4, DD254, 2 pages Attachment 3, QASP Matrix, 3 pages Attachment 2, Quality Assurance Surveillance Plan, 5 pages Attachment 1, NAVSHIPYD PTSMH INST 5090.30PNSYI 4110.1, 65 Pages N39040-16-T-0116, Solicitation, 71 Pages Non-Destructive Testing (NDT) Services, Portsmouth Naval Shipyard (PNS) located in Kittery, Maine. FAR Part 12 procedures will be used for this effort. FAR Part 13.5 procedures will be used for this effort. The Offeror shall provide Non-Destructive Test (NDT) Inspectors capable of performing Visual (VT), Liquid Penetrant (PT), and Magnetic Particle (MT), for acceptance with specified requirements. Work Environment: The offeror shall be assigned to work areas aboard a submarine. Assignments may require working extended shifts as well as weekends to support production efforts. Work may include assignments to areas where certain potentially harmful physical and chemical agents are present. These agents may include, but are not limited to; fumes, dust, noise, heat, chemicals, etc. Procedure Qualification and Approval: Nondestructive Testing (NDT) methods shall be performed in accordance with Portsmouth Naval Shipyard (PNS) written NDT procedures and Written Practice. SECURITY AND ACCESS REQUIREMENTS: Performance under this effort shall require offeror access to Restricted Data (RD), Nuclear Work Areas (NWAs), or the engineering spaces of nuclear powered vessels. Offerors shall hold a current, unexpired Facility Security Clearance. To be eligible for award, each potential offeror shall submit a Department of Defense Contract Security Classification Specification, DD Form 254, which must be approved by the Shipyard Security Office. A current, valid Facility Security Clearance for this effort is a requirement for award. Period of Performance is the base period beginning on or about 20 May 2016 through 19 May 2017 with two (2) one (1) year options.
