- Attachment 2 Government Prop Form Attachment 1 - Schedule of Prices Attachment 5 Past Performance SPECIFICATION for WILLIAM TATE RFQ_USCGC WILLIAM TATE Attachment 4 REQ for Clarification This notice replaces previously issued under Pre-solicitation No. HSCG85-16-Q-P45A20. Solicitation RFQ# HSCG85-16-Q-P45A20 The United States Coast Guard (USCG) Surfaces Forces Logistic Center (SFLC) has a requirement for DOCKSIDE REPAIRS FY 2016 to USCGC WILLIAM TATE (WLM 560). The acquisition will be solicited as a Total Small Business Set-Aside. The Coast Guard will acquire the work outlined in the solicitation utilizing procedures in Federal Acquisition Regulation FAR Part 12, Acquisition of Commercial Items, and FAR Subpart 13.5, Simplified Acquisition Procedures for Certain Commercial Items. This requirement is for contractor to provide all necessary facilities, labor, materials, equipment, supplies, power, accessories, personnel and such other things as are necessary, except as otherwise specified to perform dry-dock & repairs and alternations to the U.S. Coast Guard Cutters - USCGC WILLIAM TATE (WLM 560) in accordance with this solicitation and attachment specifications. NAICS Code/Size Standard: The applicable NAICS Code is 336611, Ship Building and Repairing and the size standard is 1,000 employees. This requirement will be evaluated using price and past performance as evaluation factors. Hard copies solicitation will not be issued. It is incumbent upon contractors to monitor the FEDBIZOPS web page for RFQ release and all subsequent amendments. PLACE OF PERFORMANCE: All work will be performed at the Home Port in Philadelphia, PA. COAST GUARD CUTTER AND VESSEL HOME PORT: 1 Washington Ave Philadelphia, PA GEOGRAPHICAL RESTRICTION: N/A PERIOD OF PERFORMANCE: Work covered by this solicitation shall be completed within 13 June 2016 through 20 September 2016 at the cutter home port. NOTICE TO OFFERORS 1. This is a Request for Quote (RFQ) for the Dockside Repairs of USCGC WILLIAM TATE, homeport in Philadelphia, PA. 2. In order to be selected for award, offerors will be required to submit written offers consistent with the requirements of this solicitation. Award will be based on evaluation factors outlined in FAR 52.212-2 Evaluation-Commercial Items. 3. Offerors must complete and submit all applicable items of this solicitation, with particular emphasis on Schedule of Items - Supplies/Services and Prices, contract clauses, and 52.212-3, Offeror Representation & Certifications - Commercial Items. 4. Offerors that elect to use commercial delivery services for delivery of their quotes are advised that such delivery must be made to Suite 800N on the 8th Floor of the Oakland Federal Building located at 1301 Clay Street, Oakland, CA by the delivery date/time specified to be considered timely. Mailed and e-mailed quotes will be accepted. Faxed quotes will not be accepted. Emailed quotes, not to exceed 10MB, may be sent to Pastaveia.St.John@uscg.mil. 5. All prospective offerors are urged to visit the cutter and visually inspect each item of work and to become acquainted with local conditions, locations of items and fixtures, removals, replacements, interference and the nature of work. USCGC WILLIAM TATE will be available for ship check during the solicitation period. Contractors are required to offer prices for all items in Schedule of Items, Attachment 1, to be considered. 7. Offerors are advised that some of the drawings, diagrams, and reports that are provided with this solicitation are for information use only. The Coast Guard does not warrant the accuracy of this information. This is the best information available to the Coast Guard and is provided to the offerors to assist them in preparing their competition quotes. 8. All questions must be submitted utilizing the Request for Clarification Form, Attachment 4, no later than 23 March 2016 at 12:00 P.M., Pacific Local Time, and be directed to: Pastaveia St. John e-mail: Pastaveia.St.John@uscg.mil. 9. REQUIRED DOCUMENTS: Each quoter shall furnish the information required by the solicitation, which includes: 1. Signed 1449 with offeror's address, DUNS and other information completed. 2. Signed Amendments, as required 3. Section B- Schedule of Supplies/ Services, filled out with a cost associated with each CLIN 4. Past Performance Documentation 5. Small Business Self Certification 6. Welding Certifications and Procedures 7. Proof of Insurance (Statement of Coverage from current Insurance carrier) 8. List the names of ALL Sub-Contractor's that your company intends to subcontract for the CLIN Item(s) listed in the Schedule of Supplies/Services. 9. **ALL¬ Technical Representative Certifications for BOTH PRIME AND SUBCONTRACTORS as specified in the specification 10. ANTICIPATED AWARD DATE: The contract is expected to be awarded on or about 3 June 2016. Work covered by this requirement shall be completed from 13 June 2016 through 20 September 2016 at the cutter home port. 11. Closing date/time for submission of offers is 31 March 2016 at 5:00 P.M., Pacific Standard Time.
