Description

Statement of Work (SOW) Price List Example Price List SOW Attachment 1 SF 1449, FA5000-16-T-0020 RFQ Joint Base Elmendorf-Richardson (JBER) has a need for Nuclear Medicines with various shelf and half-lives to be delivered to the JBER Nuclear Medicine Department. Radiopharmaceutical preparation of doses, labeling of blood and testing shall be performed at the Vendor facility and delivered to the Joint Base Elmendorf-Richardson Nuclear Medicine Department. The government intends to award a firm-fixed price Indefinite Delivery Indefinite Quantity contract. Solicitation FA5000-16-T-0020 is issued as a Request for Quotation (RFQ). This requirement is solicited under ‘Full and Open, unrestricted' competition. The North American Industrial Classification System (NAICS) code associated with this procurement is 325412 with a small business size standard of 750 employees. The Statement of Work is incorporated as Attachment 1. The Price List is incorporated as Attachment 2. Vendors shall complete the SF 1449 and Attachment 2 in its entirety and return with any other documentation/data as required. All prospective contractors must be registered and have no active exclusions in the System for Award Management (SAM) in order to be eligible for contract award. Registration may take up to three weeks to complete, so immediate attention to ensure eligibility is recommended. For registration, go to https://www.sam.gov/ and follow the instructions Questions regarding this requirement need to be submitted no later than 05 April 2016, 2:00 p.m. Alaska Standard Time.
