- I. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice and in accordance with the simplified acquisition procedures authorized in FAR Part 13. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. All offerors must also be registered on www.sam.gov. Any amendments issued to this solicitation will ONLY be available on the FBO website (www.fbo.gov ). II. This solicitation is issued as a request for quotation (RFQ). Submit written quotes on RFQ Number VA-246-16-Q-0712. This solicitation is a request for 25 Alice NightOne Wireless Sleep Testing Systems to be delivered to the Department of Veterans Affairs (VA) Health Care Center at Greenville, North Carolina which meets the requirements within this solicitation. III. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-87. IV. This is a TOTAL SMALL BUSINESS SET-ASIDE solicitation. The North American Industry Classification System Code is 334510 and the size standard is 500 employees. V. This combined solicitation/synopsis is for purchase of the following commercial supplies which meet the following salient characteristics and requirements: Alice NightOne Wireless Sleep Testing System. VI. Requirements/Salient Characteristics 1. Three sensors (effort, flow and oximetry) provide seven channels of sleep parameter data, including body position. 2. "Auto-on" feature turns on the device as soon as the belt is clicked in place; no need for patients to push a 'start' button. 3. Utilizes same sensor technology recommended by the AASM for in-lab studies. 4. The integrated connection eliminates the need for an external effort wire set. 5. Designed to prevent the need to retest due to application error. 6. Flow, effort and SpO2 signal quality is monitored throughout the night. 7. Dimensions not to exceed: 10.34 cm x 2.51 cm x 6.78 cm (4.07" L x 0.99" W x 2.67" H); weight - 84 grams (without batteries). Power requirement: alkaline or NiMH rechargeable batteries with at least 240 mAh capacity. VII. Required delivery within 30 days of award Deliver all products to the Greenville HCC, 406 Moye Boulevard, Greenville NC, 27858. VIII. FAR 52.212-1, Instructions to Offerors -- Commercial Items (OCT 2015), applies to this acquisition. IX. The Government will award a firm fixed priced contract to the responsible offeror whose offer, conforming to the requirements in this request for proposal, will be most advantageous to the Government, price and other factors considered. A best value award will be made based upon the following factors: Technical and Price. 1. Technical: ability to provide products which conform to the salient characteristics and requirements listed in this solicitation 2. Price X. The offeror must submit a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications - Commercial Items (MAR 2016), with its quote. An offeror shall complete only paragraph (b) of this provision if the offeror has completed the annual representations and certifications electronically at http://orca.bpn.gov. XI. FAR clause 52.212-4, Contract Terms and Conditions - Commercial Items (MAY 2015), applies to this acquisition. XII. FAR clause 52.211-6, Brand Name or Equal (AUG 1999) applies to this acquisition. XIII. FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items (MAR 2016) applies to this acquisition. The following clauses under subparagraph (b) apply: 52.204-10; 52.209-6; 52.219-6; 52.219-28; 52.222-3; 52.222-19; 52.222-21; 52.222-26; 52.222-36; 52.222-50; 52.223-18; 52.225-3 Alternate III; 52.225-13; 52.232-34. XIV. The following VAAR clauses are also applicable to this acquisition: 852.203-70 Commercial Advertising (JAN 2008); 852.246-70 Guarantee (JAN 2008); 852.246-71 Inspection (JAN 2008); 852.232-72 Electronic Submission of Payment Requests (NOV 2012). XV. The following VAAR Provisions apply to this acquisition: 852.273-70 Late Offers (JAN 2003); and 852.273-74 Award Without Exchanges (JAN 2003). XVI. Quotes are required to be received by the Contracting Officer electronically via email no later than 12:00 P.M. EDT on March 23, 2016. All quotes shall be emailed to darla.hurlock@va.gov. NO FAXED OR MAILED OFFERS WILL BE ACCEPTED.
