Statement of Work Equipment being ordered:_PHAROS EX 308 Excimer Laser Please provide the following information clearly and precisely. 1.Description of what the equipment being requested is: Laser excimer machine with a variable spot size handpiece and aiming beam for treatment of psoriasis and other dermatological issues. 2. Description of how the equipment being requested operates:. PHAROS excimer lasers are xenon chloride (XeCl) lasers that deliver concentrated, high-dose monochromatic 308nm UVB phototherapy directly to psoriatic and depigmented lesions limiting exposure to surrounding healthy tissue. Continuously adjustable with constant fluence, PHAROS' infinitely variable spot size handpiece and aiming beam allow targeted treatment of affected tissue quickly, easily, and accurately while limiting exposure to healthy skin. 3. Where will the equipment being requested be used: Dermatology Clinic 4. Who will be using the equipment being requested: Providers who have been properly trained in the apparatus's use for procedures as well as the cleaning and storage. 5. Justification: Currently, this treatment protocol is being fee-based out at a great cost to VA. The UVB Laser is used to treat skin lesions that are recalcitrant to other standard forms of treatment. The types of skin lesions that can benefit from this therapy include: Psoriasis Vitiligo (for which there is no other treatment) Cutaneous T-cell lymphoma Eczema Lichenified skin conditions And many others for which there are many current clinical trials
