Description
This is a Sources Sought only. The Department of Veterans Affairs (VA) is performing market research to assess the commercial marketplace for Ambulance Services at Department of Veterans Affairs, Wilkes-Barre VA medical Center (WBVAMC), 1111 East End Blvd, Wilkes Barre, PA 18711. The North American Industry Classification System NAICS is 621910 with Small Business size of $15 million. The Department of Veterans Affairs is contemplating a Firm Fixed Price, Indefinite-Delivery Indefinite-Quantity (IDIQ) 5 year contract. A DRAFT portion of the Performance Work Statement is below. The official solicitation with complete Performance Work Statement will be posted on www.fbo.gov on or before April 30, 2016. Draft portion of Performance Work Statement: 1.Purpose is to provide Non-Emergent Ambulance and Non-Emergent Wheelchair Van Patient Transportation Services for the Wilkes-Barre VA Medical Center and Outpatient Clinics. Mode of vehicle for transport is solely dependent upon Veterans ambulatory or /and medical status. Mode of transport requirements will be submitted by the VA Medical Center during scheduling. i.Allentown, Pennsylvania Community-Based Outpatient Clinic 3110Hamilton Boulevard Allentown, PA 18103 ii.Bangor Clinic 701 Slate Belt Boulevard Bangor, PA 18013-9341 iii.Berwick Community-Based Outpatient Clinic Alley Medical Center, 301 West Third Street Berwick, PA 18603 iv.Tobyhanna Community-Based Outpatient Clinic Building 220 Tobyhanna, PA 18466 v.Wilkes-Barre VA Medical Center (WBVAMC) 1111East End Blvd Wilkes-Barre, PA 18711 vi.Williamsport Community-Based Outpatient Clinic 1705Warren Avenue, Werner Building - 3rd Floor (Suite 304) Williamsport, PA 17701 vii.Wayne County Outpatient Clinic 600Maple Ave. Suite 2 Honesdale, PA 18431 2.Administration of PWS The WBVAMC will be the administrator of this PWS, and is responsible for the enforcement of this contract. All aforementioned Medical Centers and Out Patient Clinics will hence forth be referred to as "WBVAMC". Contracting Officer is the only one authorized to change the PWS via modification to the contract. 3.Hours of Service Contractor shall provide services 7 days a week, 24 hours per day, including weekends and holidays, for the term of this contract. 4.VA Patient Beneficiaries The WBVAMC will pay transportation costs only for VA patients as determined by VA regulations or in accordance with this PWS. The Contractor may contact the WBVAMC Patient Travel Coordinator or designee if there are any questions concerning Veteran's Transportations needs and requirements. 5.Requirements All work under this PWS shall be performed in accordance with any applicable US Department of Transportation or OSHA regulations, as well as applicable Pennsylvania State EMS Codified Laws and standard industry practice.
