- STATEMENT OF WORK FOR FURNISH AND INSTALL WATER SLIDES, FITNESS CENTER POOL/KIDDIE POOL AT SCOTT AIR FORCE BASE, ILLINOIS 18 March 2016 1. Description of Work: Contractor shall furnish all labor, materials, supervision, transportation, and equipment necessary to perform a turn-key project to furnish and install two new slides at building 42, Fitness Center Pool/Kiddie Pool Watnee Street, Scott AFB IL. •Purchase and install 2-tube water slide similar or equivalent to a Mason Corp Model 311 Pool Side Slide a. Install framework for the new slides and secure to the pool deck. b. Install new water slides on existing water slide site c. Connect all water and electrical requirements need to operate water slide. •Purchase and install a kiddie water slide, similar or equivalent to a Mason Corp Model 9006 Baby Duck Slide, (may propose alternate character-type slide, provide ) Site preparation will be performed by the government. 2. Accomplishment of Work: Contractor shall site verify all dimensions and /or existing site conditions prior to delivery and installation. Coordinate access to the job site two weeks prior to start of work to allow FSS to provide access. The areas being worked on must remain clear for emergency access at all times during the installation. All temporary closures, barriers, and protective procedures shall be coordinated with FSS. All work shall be accomplished between the hours of 7:30 am and 6:30 pm. 3. Performance Period: Desired turn-key completion date is 29 May 2016 after notice of contract award. Actual turn-key completion date will be as agreed prior to contract award. In the event the desired completion date cannot be met, contractor will be required to propose an alternate installation schedule to allow normal pool operations to continue. 4. Work Schedule: The contractor shall coordinate work with the designated FSS representative. The Contractor shall coordinate with the project manager/inspector and notify FSS when on-site work is to commence. Contractor shall also provide a work schedule and updates if required. This schedule shall describe major work items and projected completion dates. 5. Protection of Building Property: The Contractor shall take necessary measures to protect all property located in or adjacent to the project site and shall repair, replace or otherwise be held liable for all such damage. 6. Cleanup: The contractor shall cleanup each work site at the end of each work day to the satisfaction of the FSS. Upon completion of the work, all construction equipment and debris shall be removed from the site and the area shall be left in a neat and completely acceptable condition as required. 7. Patch and Repair: After the work has been completed, any holes, cuts, and damages made in the finished areas of the work site and adjacent areas shall be properly repaired and restored to match the existing finishes. 8. Products: All products shall conform to their respective manufacturing and performance publications. All materials and equipment used in the work area shall be the products of manufactures regularly engaged in the manufacture of construction products. 9. Specifications: Work shall be performed in strict accordance with this contract's general specifications, as well as all applicable local, state, and national building and life safety codes, American Disability Act regulations, Illinois Department of Transportation specifications for Roadways Act regulations, Illinois Department of transportation Specifications for Roadways and Bridges, Safety issues must adhere to all applicable Occupational Safety and Health Act regulations, to include all other issues listed in the us Army Corps of Engineers Safety and Health Requirements manual (EM 385-1-1).
