- Attachment A U.S. Army Engineer Research and Development Center (ERDC) Sources Sought Notice - Lab Testing Services The US Army Corps of Engineers (USACE), Engineering Research and Development Center (ERDC) is conducting a sources sought to obtain information regarding: (1) the availability and capability of qualified small business sources; (2) whether there are Service-Disabled Veteran-Owned, Veteran-Owned, HUB Zone, 8(a), Women-Owned, or small disadvantaged business concerns; and (3) their size classification relative to the North American Industry Classification System (NAICS) code for the proposed acquisition. Your responses to the information requested will assist the Government in determining the appropriate acquisition method, including whether a set-aside is possible. An organization that is not considered a small business under the applicable NAICS code should not submit a response to this notice. The NAICS code is 621511 (Size Standard: $32.5m) The Government will not pay any costs for documentation submitted in response to this Sources Sought. Potential offerors are invited to provide a capabilities statement demonstrating ability to provide the requested services via e-mail to kisha.m.craig@usace.army.mil. The Capability statements must include the following: 1) Company name and Point of Contact information (address, telephone, e-mail, web-site, etc.); 2) DUNS number and CAGE code; 3) Small Business Size/Certification (to include 8(a), HUB Zone, SDVOSB, etc.); and (4) technical capabilities. The U.S. Army Engineer Research and Development Center (ERDC) is seeking a vendor to provide Lab Testing Services. Refer to the General Requirements section below for the requested services description. This Sources Sought Notice is designed to facilitate the Contracting Officer's review of the market base, for acquisition planning, size determination, and procurement strategy. Definitions: • Routine Test - A test that is usually performed in which the result is generally required within 24 hours. • TAT- The length of elapsed time between receipts of the specimen at the contractor's laboratory until the receipt of the completed printed report back in the clinic. • Test Result Report - A printed final copy of laboratory testing results. • POC- Point of Contact GENERAL REQUIREMENTS Services are required to perform select lab testing services, for the U.S. Army Engineer Research and Development Center (hereinafter referred to as ERDC). This contract shall cover lab testing services. The services would be provided in a state-of-the-art civilian medical facility. The standard of services shall be of quality; meeting or exceeding those outlined in the licensing and accreditation section of this PWS. Contactor policies and procedures shall comply with Health Insurance Portability and Accountability Act (HIPAA). The contractor shall provide the following services and supplies: I. Centrifuge machine to be stored and operated at the ERDC. II. Specimen Collection Box to be installed and used at the ERDC. Contractor is responsible for disposing of the specimen collection box in accordance with all local laws. III. Blood drawing equipment (including all blood drawing tubes and needles, butterflies, specimen bags, urine cups, 24 hour urine jugs) to be stored, stocked and used at the ERDC IV. Secured results printer & supplies (if not available on the web site) to be stored, stocked and used at the ERDC. Results must be secured in accordance with the HIPPA Privacy Rule (45 CFR Part 160) V. Labs MUST be picked up by 3 pm central standard time each day. VI. Shall provide select lab testing services: A. Contractor is required to perform analytical testing for ERDC patients for the tests defined on the list in Attachment A. B. The Contractor is required to provide a report of analytical test results and consultative services as required assimilating the full scope of its laboratory operations. Reporting of Results: 1. Test result reports will be supplied to ERDC for entry into the patient's chart. C. The contractor shall provide a Laboratory Manual (or have available on the website) to include testing methodology and containing the following information for testing services: a. Department hours of operation b. Accreditation: CAP & CLIA c. Technical Staff d. Service Departments (method of contacting, phone numbers, hours of availability) e. Quality Assurance Information f. Billing Procedures & fee schedules for services provided g. Procedures and criteria for phoning reports and other important information h. Report forms: 1) The contractor shall provide telephone number(s) and contact person(s) available to ERDC to make specimen problem inquiries and problem solving; 2) contractor agrees to maintain the minimum acceptable service, reporting systems, and quality control as specified herein. Immediate (within 24 hours) notification must be given to ERDC upon adverse action by a regulatory agency; 3) Contractor shall assign a specific local account representative; 4) Contractor shall advise facility of any changes in methodology, procedure or reference ranges; 5) Shall perform testing as needed 24 hours per day/7 days per week; 6) The Contractor shall carry out its functions hereunder in full compliance with all local, state, and federal laws or regulations; 7) The contracting laboratory shall not release patient's records that include test results, without the specific written consent to release such information as provided by the patient, to any person other than the ordering healthcare provider or designee. All member records shall be treated as confidential so as to comply with all state and federal laws regarding the confidentiality of patient's records. This provision shall survive termination of the resulting contract award; 8) Shall certify and ensure that all employees, officers, or agents do not use Protected Health Information received from ERDC that would constitute a violation of any applicable provision in standards set forth in the Health Insurance Portability and Accountability Act (HIPAA). Transaction Location: 1. U.S. Army Engineer Research and Development Center, 3909 Halls Ferry Road, Vicksburg, MS 39180-6199. LICENSING AND ACCREDITATION: A. Shall have all licenses, permits, accreditation certificates required by Federal law in and State law in Mississippi. B. Shall be accredited by the Department of Health and Human Services Clinical Laboratory Improvement Act of 1988 (CLIA) or the College of American Pathologists (CAP). Copies of all professional certifications, licensures and renewal certifications will be provided and updated as needed to the Contracting Officer to include the contracting laboratory's Laboratory Director and/or Medical Director. C. Medical Director must have suitable qualifications and experience to direct a laboratory providing consultation services under this contract according to CLIA and CAP standards. D. Contractor Personnel assigned by the Contractor to perform the services covered by this contract shall be eligible to provide the services of this contract and licensed in a State, Territory, or Commonwealth of the United States or the District of Columbia. All licenses held by Contractor personnel working on this contract shall be full and unrestricted licenses. Contractor Personnel assigned by the Contractor to work under this contract shall be licensed by the governing or cognizant licensing board. E. Must comply with the regulatory requirements of Health and Human Services Health Care Financing Administration. F. Must notify the Contracting Officer immediately, in writing, upon its loss (or any of its subcontractors) of any required certification, accreditation, or licensure. This is NOT a Request for Proposal and does not constitute any commitment by the Government. Responses to this sources sought notice will be used by the Government to make appropriate acquisition decisions. All interested sources must respond to future solicitation announcements separately from responses to this market survey. Responses to this sources sought announcement are required not later than Mar 28, 2016 by 5pm CST to kisha.m.craig@usace.army.mil.
