Description
REQUEST FOR INFORMATION / SOURCES SOUGHT INQUIRY The Department of Veterans Affairs Network Contracting Office (NCO) 19, Rocky Mountain Acquisition Center (RMAC) is seeking information and sources that are capable of providing temporary locum tenens Optometrist support to the Southern Colorado region VA Eastern Colorado Healthcare System (VAECHCS) at the following VAECHCS clinics: 1.PFC Floyd K. Lindstrom Outpatient Clinic 3141 Centennial Boulevard Colorado Springs, CO 80907 2.Pueblo Outpatient Clinic 4776 Eagleridge Circle Pueblo, CO 81008 Qualifications: The qualifications of such personnel shall be subject to review by the VAECHCS Chief of Staff and approval by the VAECHCS Facility Director. The Locum Tenens Optometrist may be replaced if not performing up to expectations. Optometrist shall perform all duties associated with being a staff Physician. Duties include, but are not limited to: a.Patient examinations (new follow-up and consultative). b.Obtain patient consent prior to procedure via iMED Consent. c.Receive calls from other providers regarding their patients. d.Procedures shall be within the Optometrist's approved privileges. e.Provide ongoing outpatient care for stable health problems. f.Exams appropriate and specific for Optometric services for outpatient, inpatient, and Community Living Center (CLC) resident examinations. g.Screening outpatients to determine their need for further care. h.Following Postoperative patients after discharge, determining need for further follow-up care and arranging for care. i.Ordering of routine laboratory, radiologic, and other diagnostic tests. j.Initiating and expediting request for consultation, scheduling special tests and studies. k.Documentation in medical record, changing orders when appropriate. l.Writing prescriptions: Physician shall use VAECHCS Formulary, and Formulary shall be provided to the successful candidate. m.Perform procedures within VAECHCS's level of complexity. n.Progress notes and outpatient encounters shall be completed the same day of the patient's visit. Period of Performance: The anticipated period of performance shall be from 01 April, 2016 through 31 November, 2016; an estimated 180 days. The period of performance may be extended up to six (6) months if staff cannot be recruited and hired by VAECHCS within this time period. Work Schedule: This schedule is variable and the Optometrist may be required to work variable hours and/or extended coverage hours which could include Saturdays. Monday through Friday - 8:00 AM to 4:30 PM, Monday through Friday evening hours - 4:30 PM to 6:30 PM Saturdays - 8:00 AM to 2:00PM. The information submitted shall contain the company's business size status. This is a request for information and sources only, which may or may not lead to a future solicitation! This is not a request for proposal (RFP). The VA will not pay for any information received resulting from this sources sought notice. Requests for copies of a solicitation shall not be honored or acknowledged. Information should be forwarded to the Contract Specialist.
