Description

This is a combined Synopsis/Solicitation for commercial services/items prepared in accordance with the format in FAR Subpart 13.5, as supplemented with additional information included in this notice. The solicitation number is VA250-16-Q-0194 (Renovate Research North Commissioning Services) and is issued as a Request for Quote (RFQ), unless otherwise indicated herein. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-69. The associated North American Industrial Classification System (NAICS) code for this procurement is 541330. This requirement is a Service Disabled Veteran Owned Small Business set-aside and only qualified contractors may submit quotes. The VHA NCO 10 - 541 - Louis Stokes VA Medical Center requires: Commissioning Services for the Renovate Research North project. The requested items are specified below: 1. The Commissioning Agent (CxA) shall perform all Design Phase, Pre-Bid Phase, Construction Phase, Acceptance Phase and Warranty Phase commissioning services as an independent Commissioning Agent of the Veterans Affairs, for VA Project No. 541-13-104, "Renovate Research North" at the Wade Park Campus of the Louis Stokes Cleveland VAMC, Cleveland, Ohio. 2. The CxA shall perform these services in FULL ACCORDANCE WITH the Department of Veterans Affairs "Whole Building Commissioning Process Manual" and TIL - Master Construction Specifications (PG-18-1), Section 01 91 00 - General Commissioning Requirements. Commissioning Services may include, (but may not be limited to) the services outlined below. Refer to the "Whole Building Commissioning Process Manual" for a complete description of the Commissioning Services to be provided. The manual is available at: http://www.cfm.va.gov/til/Cx-RCx/CxManual.PDF. 3. See attached Statement of Work for the major scope items for this design-bid-build construction project. ***Request for Information Submissions: Interested contractors must submit questions, via e-mail, concerning the solicitation NLT APRIL 27, 2016 to enable the Contracting Specialist and/or COR to respond. Questions not received within a reasonable time prior to close of the solicitation may not be considered.*** Quote MUST be good for 120 calendar days. The selected contractor must comply with the following commercial item terms and conditions, which are incorporated herein by reference: FAR 52.212-1, Instructions to Offerors - Commercial Items; FAR 52.212-3, Offeror Representations and Certifications - Commercial Items - the selected offeror must submit a completed copy of the listed representations and certifications; FAR 52.212-4, Contract Terms and Conditions - Commercial Items; FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, paragraph (a) and the following clauses in paragraph (b): 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.225-13, 52.232-34. In addition to any VHA terms, the contractor has reviewed and acknowledges any attached solicitation documents, item details, delivery schedule, and/or instructions (collectively referred to as requirements), and agrees to all terms and conditions set forth in the solicitation documents. While additional support documentation (e.g. technical drawings, etc.) may be requested, pricing shall be entered via e-mail only - verbal pricing is not an acceptable Quote.
