Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be issued. Solicitation number VA248-16-Q-0813 is issued as a Request for Quote (RFQ). This solicitation documents and incorporates provisions and clauses in effect through Federal Acquisition Circular 2005-87-1, effective March 16, 2016. This solicitation starts 04/15/2016 and will end 04/20/2016 at 2:00 pm EST. Network Contracting Office (NCO) 8 is seeking qualified small business contractors to provide four-strap system (two on each side) Rehab Back Braces which provides maximum lumbar support for patients of all heights for the Miami VA Healthcare System. This is a brand name or equal requirement (Stealth and Vertzmax brands). See attached and below schedule for equal standards and salient characteristics This solicitation is set-aside for small business Contractors. The associated North American Industrial Classification System (NAICS) code for this procurement is 339113, with a small business size standard of 500 employees. This will be a Firm Fixed Price Contract. The government anticipates awarding a single award contract. All qualified small business sources are encouraged to submit a Quote. Questions should be submitted via e-mail to melva.stennis@va.gov The salient characteristics of the products being requested are as follows: "Front and back panel designed to produce compression to reduce intervertebral discs load to the lumbar region "Corset with eight (8) flexible stays "Removable Front Polymer Panel and Rehab Back Polymer Panel "Lumbar support for patient of all heights "Anterior Panel and removable Rehab Panel "Anterior and posterior panels should automatically contour to patients shape, size and lordotic curve "Durable interlocking posterior straps "Undetectable under clothes "Comfortable, breathable, cotton fabric Please follow all instructions given in attached Solicitation #VA248-16-Q-0813 and fill out the price schedule on pages 6 and 7, sign the Solicitation document (See Item Number 30a and 30b) and return a signed copy before due date and time (see line item 8) on the front page.
