Description

This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotes are being requested and a written solicitation will not be issued. The solicitation number is VA257-16-Q-0991. This acquisition is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-87. The associated North American Industrial Classification System Code (NAICS) for this procurement is 334510 with a small business standard of 1,250 employees. This acquisition is solicited as a set-aside for small business. Network Contracting Office 17 requests responses from qualified sources capable of providing the following item(s): DescriptionQty CARTO 3 System CONFIDENSE Mapping Module This module provides rapid acquisition of mapping points based on physician standards. Included are the capability to continuously acquire MEM points, indicators of tissue proximity for MEM-enabled catheters, advanced electrogram annotation algorithms, and advanced map integrity algorithms1 The following FAR and VAAR Clauses and provisions in their latest editions apply to this solicitation. 52.212-1, Instructions to Offerors-Commercial Items. 52.212-2, Evaluation Commercial Items. Technical acceptability to meet the Governments requirement, New equipment only. No remanufactured or gray market items. All items must be covered by the manufacturer's warranty. Vendor shall be an original equipment manufacturer (OEM) authorized dealer, authorized distributor or authorized reseller for the proposed equipment/system such that OEM warranty and service associated with the equipment/system shall be in accordance with the OEM terms and conditions. Submit original documentation from the manufacturer stating authorization of dealership for the item being procured; Past performance, delivery and price.
