Description

Request for Information This is a request for information, in order to determine the commercial availability of the services described in the draft statement of work below. This posting is for informational purposes only and shall not be construed as an official request for quote or request for proposal. The purpose of this request is to gain information regarding potential sources and vendors for the services described below. In response to this request, please provide a statement of capabilities that shows the respondent is capable of meeting the requirement outlined in the draft statement of work. Submittal of pricing information, quotes, or proposals is not required at this time, and any requests for such shall be submitted via separate request for quote. In addition to a capabilities statement, vendors should provide past performance information, to include a government point of contact, which demonstrates the vendors successful performance of these services, currently, or in the past. Draft Statement of Work (SOW) "Purpose oThe purpose of this effort is to enter a short term rental agreement (6 months, with and option for an additional 6 months) with a contractor to provide a mobile Audiology Unit equipped with the necessary instruments, capacity, and characteristics to conduct diagnostic audiological evaluations. The medical center faces a considerable backlog of consults for incomplete Audiology exams that have accumulated over the past five years. The backlog is due to a prolonged absence of the specialized physician services and the facility's limited availability and access to functioning audiology booths. The short term rental of the mobile Audiology unit will enable the facility to process the more than 600 hearing evaluations and greatly improve or eliminate the wait time for care for our Veterans. "Scope oThe contractor shall provide a mobile Audiology Unit for rent 24 hours a day, seven days a week to the Carl Vinson VA Medical Center for a period of six (6) months with a six (6) month option to rent. oThe unit shall include a wheel chair access ramp for non-walking patients. "Mobile Audiology Unit Site oThe contractor shall deliver the mobile Audiology Unit to the medical center property and locate/set-up the unit on a space designated by the facility engineering service line point of contact. oThe designated location shall be coordinated and agreed upon after the contractor has conducted a site visit to the Carl Vinson VA Medical Center campus. oThe medical center will provide a 220V 50 Amp Twist Lock Outlet as the power source for enabling the unit. "Mobile Audiology Unit Liability Coverage oThe contractor shall be responsible for all necessary insurance coverage for any damage to the unit's components and accessories and all other liabilities related to damage of the unit as a result of this contract. "Statement of Required Goods/Deliverables oMobile Audiology Unit Characteristic Specifications: "8-12 Patient Capacity "AudioScan-Verifit (Real Ear Hearing Aid Analyzer) Model #VF-1 "Minimum software compatibility of 3.10.27 July 10, 2013 or higher "Interacoustic AAA audiometer "40x17 ft. "Handicap Accessible Ramp "220V 50 AMP Power "Reception Area "Clinical Office with isolation booth "Double Walled booth "Tasks oThe contractor shall be responsible for all mobile Audiology unit maintenance, repair, calibration and general housekeeping during the term of the rental agreement. oIn the event of any equipment malfunction, the contractor shall respond to and complete all repairs within 24 hours. oThe contractor shall provide initial familiarization training during deliver and set up. oThe contractor shall also provide 24 hour telephone technical support and troubleshooting assistance on minor system issues throughout the term of the contract.
