This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is VA250-16-Q-0319 and is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-87. The associated NAICS code is 339113Surgical Appliance and Supplies Manufacturing and small business size standard is 500 employees. This is a total small business set-aside. The requirement is for the acquisition of Red Biohazard Surgical Instrument Containers Healthmark Industries Brand Name or Equal. Delivery location is the Louis Stokes, Cleveland VA Medical Center, 10701 East Boulevard, Cleveland, OH 44106-1702. This announcement constitutes the only solicitation; a formal, written solicitation will not be issued. PAPER COPIES OF THIS SOLICITATION WILL NOT BE AVAILABLE. This solicitation documents and incorporates provisions and clauses in effect through FAC 2005-87. During this inspection, they determined that SPS needed to provide more Red Biohazard Surgical Instrument Containers in order to transport Dirty/Contaminated instruments from Clinics to SPS. National Program Office for Sterile Processing also requested that all reprocessing areas have appropriate PPE Signage displaced on all entry and exits. Specifications: Red Biohazard Surgical Instrument Containers from Healthmark Industries Brand Name or Equal. Must be able to contain critical/semi-critical reuseable medical equipment in order to minimize airborne/contact spread of microorganisms and reduce the risk of cross-contamination and infection All containers must be labeled with biohazard and the biohazard symbol, Red or blue in color, made of medical grade/puncture resistant polycarbonate that can be properly cleaned and decontaminated All containers must be rigid not flexible in nature All containers must include lids with latches to firmly seal the transportation unit All carts must be closed transport carts must contain doors with hinges and latches that firmly seal the cart from potential cross-contamination during transport. PPE Signage must contain vivid graphics to grab the attention of VAMC employees that are not aware of PPE Standards. EVALUATION CRITERIA/ SUBMISSION OF OFFER All Questions Shall be submitted in writing in response to the Solicitation and are due no later than Wednesday 1 June 2016 at 3:00 PM EST. In addition to price, offers were be evaluated on technically meeting the Governments requirement, and delivery schedule. In addition to completing the cost/price schedule, contractors shall provide sufficient data/brochures to allow the Government to complete a technical comparison of the items offered to meet the Government's requirements. Failure to do so will result in an unacceptable offer. A proposed delivery schedule is to be provided.
