STATEMENT OF WORK James J. Peters VA Medical Center 130 West Kingsbridge Road Bronx, NY 10468 A.GENERAL INFORMATION 1.Title of Project: Renal Biopsy-Fresh Tissue (transplant recipient) Processing, Staining, IHC (Immunohistochemistry), EM (Electron Microscopy) & pathology analysis to be performed at Icahn School of Medicine, Dept. of Pathology at Mt. Sinai Medical Center. 2.Background: Renal Transplant Services requires renal biopsy processing for special stains, IHC, & EM and analysis by renal pathologist. This service is required for transplant recipient patients management. 3.Performance Period: The contractor shall provide the services required under this Statement of Need on the day/time renal biopsy tissue is received at the Icahn School of Medicine, Department of Pathology. 4.Type of Contract: Firm Fixed Price at approved Medicare rates. 5.Contracting Office Address: VISN 3 Network Acquisition & Logistics James J. Levy Place, Box 1194 New York, NY 10029-8014 Phone: 212-241-8014 Fax: 212-828-4188 6.Statement of Work: Fresh renal tissue obtained surgically from a transplant recipient will be picked up at the JJP VAMC by the current courier used by Mt. Sinai. Designated pick up locations are: a.)Histology Laboratory, room 2B-37-100, Monday- Friday, 8:00 AM to 4:30 PM. b.)Specimens obtained on Saturday or after 4:30 PM will be picked up in the Core Laboratory, room 2B-15-100. Renal biopsy tissue from the transplant recipient will be processed and slides will be made and stained for light microscopy, immunohistochemistry, & electron microscopy. When renal tissue is received before 12:00 PM at the Icahn School of Medicine, Dept. of Pathology, a preliminary analysis (report) will be available on the same day, between the hours of 4:00 & 6:00 pm. If the sample is received after 12:00 PM, the preliminary analysis (report) will be available the following day. Renal tissue received on Saturday or after hours (emergency) will incur additional charges for professional fees (Pathologist's analysis). The number of biopsies to be sent for processing & analysis has been estimated at 5 for base year period from 7/1/16 to 9/30/16 and 20 per year for each option year beginning 10/1/17 & ending 9/30/20.
