REQUEST FOR QUOTE (RFQ) STATEMENT OF WORK (SOW) THIS IS A COMBINED SYNOPSIS SOLICITATION REQUEST FOR QUOTATION FOR COMMERCIAL ITEMS R.F.Q. #CF4186301 ISSUE DATE: 06/14/2016 FOR: Chair Re-Upholstery Issued By: Christopher Fisher, Contracting Officer email: Christopher.fisher@nih.gov Telephone: (919) 541-0428 NATIONAL INSTITUTE OF ENVIRONMENTAL HEALTH SCIENCES OFFICE OF ACQUISITIONS ALL QUOTES SHALL BE DELIVERED VIA E-MAIL TO : Christopher.fisher@nih.gov ISSUED AS 100% Small Business Set-Aside QUOTATION DUE NO LATER THAN 1:00 p.m., JUNE 20, 2016 SUPPLIES AND PRICES Statement of Work (SOW) Contractor shall provide services for the removal of existing KI- Piretti Stack Chairs, recovering of the existing upholstery, and re-deliver in completed phases as further described below. This project is in conjunction with a refurbishing all the seating in the Rodbell conference room in B1 module in the NIEHS, Rall Bldg. on the NIEHS campus located at 111 TW Alexander Drive. For the purpose of this project, all services shall occur off site. All of the chairs will need to be picked up in the lobby of the Rodbell Conference room on the NIEHS campus located at 111 TW Alexander Drive. Services include evaluating existing conditions of Piretti Stack chairs; coordinating the removal of batches of chairs in phases, recovering the entire batch, allowing any off gassing time, and returning completed batch and pick up a new batch required for each phase. 48 Chair should be released in 5 batches, totaling 240 Chairs in all. Contractor shall meet with the Project Officer (PO) as needed to discuss project requirements. In conjunction with the PO and the NIEH Facility Liaison, the contractor shall coordinate the Phasing schedule to correspond with the Master calendar schedule for the Rodbell conference Room. The contractor shall purchase and install the fabric selected and approved by NIEHS, fabric specifications and selection is listed below. Contractor must notify the PO as soon as the fabric is received. Chair recovering and off gassing time is to not exceed 12 days in each phase/batch. Contractor will repair any damaged foam cushions, scuffs marks, and loose parts to the extent possible. Any unrepairable chairs should be called to the attention of the PO. The fabric must be adhered tightly with no puckers or loose patches. All work will be done offsite and the contractor shall assume a reasonable amount time to off-gas any adhesive fumes. Fabric Specifications are listed below: • Fabric Selected: Arc Com • Pattern: Riviera • Color: Avocado #2 • Number: AC-68871   Proposed Schedule Schedule will be coordinated upon of receipt of fabric in coordination with the PO, the Master Calendar, and room use of the Rodbell Conference room. Additional Project Information: All of the chairs will need to be picked up in the lobby of the Rodbell Conference room. All chairs maybe loaded and unloaded at the C loading dock. Quality Assurance Surveillance Plan: Contractor shall ensure the following: • Batches of 48 chairs must be able to be completed in 5 phases, and within 10-12 days business days. • Provide continuous inspections of conditions of chairs and workmanship of reupholster. • Provide minimal repair of any damaged foam cushions, scuffs marks, and loose parts to the extent cost value of replacing the chair. • Provide 100% review and satisfactory resolution of customer/end-user complaints/trouble reports submitted to PO. Basis for Award: The Government to award an order to the responsible Contractor whose offer represents the best value to the Government. The basis on which award will be made is price and other factors. Technical factors are significantly more important than price. Award will be in the aggregate to the responsive offeror fully meeting the mandatory technical factors listed below. Offeror must submit information to evaluate their offers Failure to provide the information required to evaluate the offer may result in rejection of that offer without future consideration. Technical Evaluation Factors: 1. Contractor must be capable to reupholster KI Pirettis to manufacture's spec. 2. Contractor shall provide a minimum of two references from recent projects relevant to the scope. 3. Contractor must be able to be flexible in scheduling of phases in the event we need to space out the dispatching of a phase.
