Description
ATTACHMENT - List of Provisions and Clauses Applicable to this RFQ DHS, FEMA, MWEOC Emergency Services Division Fire Department Turn-out Gear Standards This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 and FAR Subpart 13.106 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This combined synopsis/solicitation is being issued as Request for Quote (RFQ) No. HSFE50-16-Q-0002, which will result in an award for the purchase of 33 sets of firefighter turn-out gear (set = 1 jacket and 1 pant). These turn-out gear sets will be delivered to the Department of Homeland Security (DHS), Federal Emergency Management Agency (FEMA), Mount Weather Emergency Operations Center (MWEOC), 19844 Blue Ridge Mountain Road, Mt. Weather, VA 20135-2006. The following line items are required: Item No. 0001, 33 each, firefighter turn-out gear jackets in accordance with the attached standards; Item No. 0002, 33 each, firefighter turn-out gear pants in accordance with the attached standard. Item standards are provided in the attachment titled "DHS, FEMA, MWEOC Emergency Services Division Fire Department Turn-out Gear Standards". Offerors are required to submit individual prices for the items listed above, as well as descriptive literature and specification sheets for the products being offered in your proposal. This information is required in order for the Government to perform an evaluation of your proposed product to determine if it will meet the minimum requirements as stated in the attachment standards. Failure to provide this information may result in rejection of your proposal. In addition, the successful offeror must be located within 100 miles of the DHS, FEMA, MWEOC facility to accommodate making alterations, repairs, replacement and sizing of employees. The offeror must be able to provide sample gear and travel to the MWEOC facility to measure all firefighters in order to ensure the proper fit of all turn-out gear. FOB Destination pricing is requested for both items. Please include all applicable Government discounts in your offer. FEMA is a tax exempt agency; please do not include sales tax on your quote. The RFQ clauses and provisions are those in effect thru Federal Acquisition Circular 2005-88, effective 6/15/2016. This acquisition is being solicited as a total small business set aside. The NAICS code applicable to this requirement is 339113; small business size standards are 750 employees. Only one award will be issued as a result of this solicitation via the issuance of simplified acquisition/commercial item purchase order or a delivery order under a GSA Schedule contract. FAR Provisions 52.212-1, 52.212-2, 52.212-4 and 52.212-5 apply to this acquisition. Offerors are to include a completed copy of FAR provision 52.212-3 "Offeror Representations and Certifications--Commercial Items" with their response or provide this information via the System for Award Management (SAM) website, https://www.sam.gov/portal/public/SAM/. The successful offeror must provide their company DUNS number which is registered and active in the System for Award Management (SAM). A list of provisions and clauses applicable to this RFQ is provided as an attachment to this combined synopsis/solicitation. Reference the attachment titled "List of Provisions and Clauses Applicable to this RFQ". BASIS FOR AWARD : The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. In order to be considered technically acceptable, the contractor must provide the following: • descriptive literature and specification sheets for the products being offered; those products being offered must meet most current NFPA 1971 Standards for Structural Firefighting, as well as the standards listed in the attachment titled "DHS, FEMA, MWEOC Emergency Services Division Fire Department Turn-out Gear Standards"; • evidence that they are located within 100 miles of the MWEOC facility and are able to provide sample gear and travel to the MWEOC facility to measure all firefighters for the proper fit of the turn-out gear; • evidence that it is a small business and their DUNS number is actively registered in the System for Award Management (SAM); QUESTIONS : Questions regarding this RFQ are to be submitted in MS Word format to Phyllis.Woodward@fema.dhs.gov no later than 4:00 PM Eastern on July 8, 2016. Hand deliveries and/or faxes of questions will not be accepted without prior approval. Additionally, the subject line of your email shall, at a minimum, contain the RFQ number (HSFE50-16-Q-0002). All questions pertinent to the submission of a proposal in response to this RFQ will be addressed in an amendment to this RFQ. Sources of the questions submitted will not be provided. PROPOSAL SUBMISSION INSTRUCTIONS : Offerors shall submit an electronic proposal via email to Phyllis.Woodward@fema.dhs.gov no later than 12:00 PM Eastern on July 18, 2016. The subject line of emails containing an offer in response to this RFQ must clearly reference RFQ # HSFE50-16-Q-0002. Contractor proposals shall be valid for 30 calendar days after the due date for responses to this RFQ. Total size of email proposals shall not exceed 25MB due to DHS email restrictions. Hand deliveries and/or faxes of proposals will not be accepted without prior approval. A written notice of award or acceptance of proposal, emailed or otherwise furnished to the successful offeror shall result in a binding contract without further action by either party. Amendments to this RFQ will be published in the same manner as the initial synopsis/solicitation.
