- Description
- This notice serves to announce the intent of the 81ST Contracting Squadron to issue a solicitation for personnel to support a special Photo-Refractive Keratectomy/Laser-In-Situ-Keratomileusis/ Implantable Collamer Lens (PRK, LASIK, and ICL) program, at the 81st Medical Group, Keesler Medical Center, Keesler Air Force Base, Mississippi (MS). The contractor shall furnish all labor, tools, materials, transportation and all else necessary to provide this service. The period of performance will be 1 Oct 16 - 30 Sep 17, plus four one-year options for a total performance period not to exceed 66 months. The Performance Work Statement will be posted with the solicitation. The solicitation will be uploaded to www.FedBizOpps on or about 28 Jul 2016. Solicitations should be sent to: 310 M Street, Bldg 4605 Rm 102, Keesler AFB, MS 39534-2701, Attn: Paul Montgomery. This acquisition is being offered for competition limited to 8(a) concerns. The associated North American Industry Classification System for this procurement is 621320 and the small business size is $7.5 million. The requirement will be solicited as a commercial service using Simplified Acquisition Procedures under the authority of FAR 13.5. It is the potential offerors responsibility to monitor the website for any postings and/or solicitation changes, amendments and closing date. No paper copies will be available and no electronic or verbal responses will be accepted. Interested parties may identify their interest and capability by responding to the solicitation by delivering one hard copy of a signed proposal with signed amendments (if any) by the posted close date and time. Solicitation and all amendments (if any) must be submitted via mail with wet ink signature, NO electronic form of transmission (which includes, fax and email) will be accepted. The solicitation may be downloaded from FedBizOpps, since copies of the solicitation will not be available for mailing. All prospective offerors must be registered in the System for Award Management prior to award of contract with any DOD agency. Registration may be accomplished at https://www.sam.gov/. The government anticipates award of a firm-fixed price contract as a result of the solicitation.
