- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Requirements (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation will not be issued. The solicitation number for this requirement is 2016-Q-65605 and is hereby issued as a Request for Quote (RFQ) using FAR Subpart 13.1 Simplified Acquisition Procedures. The Centers for Disease Control and Prevention intends to award a firm-fixed-price contract for the following requirement, please either fill in the dollar amounts or provide a separate excel spreadsheet containing the same information including a list of line item number(s) and items, quantities, units of measure, and options, if applicable: CLIN 0001 MiniCollect® 1ml K3-EDTA Blood Collection, Greiner Bio-One 1ml 100 Pack QTY NEEDED 30 PACKS CLIN 0002 ALCOHOL SWABS, BD TM 1200 CASE QTY NEEDED 30 CASES CLIN 0003 MEDIPLAST ADHESIVE BANDAGES 100 PACK QTY NEEDED 30 PACKS CLIN 0004 BD MICRO-FINE TM CONTACT-ACTIVATED LANCET, 2.0MM; 1.5MM WIDTH BLADE 200 PACK QTY NEEDED 150 PACKS Your quote shall include the following: • Line item pricing for CLINs 0001, 0002, 0003 and 0004 • Total Price for CLIN 0001, CLIN 0002, CLIN 0003 and CLIN 0004 Delivery Date: 08/31/2016 Delivery Address: CDC DISTRIBUTION CENTER (WAREHOUSE) 3719 NORTH PEACHTREE RD. CHAMBLEE, GA 30341-2221 Acceptance and FOB point The quote format is at the discretion of the offeror. It is the offeror's responsibility to be familiar with the applicable clauses and provisions. Clauses and provisions may be accessed via the Internet at website http://farsite.hill.af.mil. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-85 (February 26, 2016). A Firm-Fixed Price purchase order will be issued in writing to the successful offeror.
