Description

This is a Combined Synopsis/Solicitation for Commercial Items prepared in accordance with the format in FAR Subpart 12.6. Streamlined Procedures for Evaluation and Solicitation for commercial items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a copy of the solicitation is attached. The Department of Veterans Affairs (VA) intends to award a Firm Fixed Price Blanket Purchase Agreement (BPA) in accordance with FAR Part 13.303 with a 12-month ordering period for the VA Office of Procurement and Logistics on behalf of Veteran Health Administration. COMBINED SYNOPSIS/SOLICITATION i.This is a Combined Synopsis/Solicitation for Commercial Items prepared in accordance with the format in FAR Subpart 12.6 Streamlined Procedures for Evaluation and Solicitation for commercial items, as supplemented with additional information included in this notice. ii.This announcement constitutes the only solicitation; quotes are being requested and a copy of the solicitation is attached. iii.Solicitation number VA119-16-Q-0484 is issued for this combined synopsis/solicitation; this solicitation is being issued as a Request for Quote (RFQ). iv.This combined synopsis/solicitation incorporates provisions and clauses in effect through Federal Acquisition Circular (FAC) 2005-89 dated July 14, 2016. v.This solicitation is issued pursuant to full and open competition procedures under North American Industrial Classification System (NAICS) code 325320 with a size standard of 1,000. vi.Schedule B of the SF1449, (Schedule of Supplies/Services) contains a list of contract line items to be included in the kit, projected quantities, and units of measure. vii.Date(s) and place(s) of delivery (VA Enterprise Wide) and acceptance are to be determined FOB destination. viii.52.212-1 Instructions to Offerors-Commercial is applicable to this acquisition and the addenda in Attachment C - Instructions to Offerors applies; ix.52.212-2 Evaluation-Commercial Items is applicable to this acquisition and the addenda are detailed in Attachment D - Evaluation of Quotes. x.Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. xi.The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition. xii.The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders -- Commercial Items, applies to this acquisition and a statement regarding which, if any, of the additional FAR clauses cited in the clause are applicable to the acquisition. xiii.Additional contract requirement(s) as well as terms and conditions are determined by the contracting officer to be necessary for this acquisition and are consistent with customary commercial practices. Please refer to Attachment B - Invoicing Instructions for VHA Contracts. Attachment A - Administrative Data and BPA Terms and Conditions and Attachment E - BPA Quarterly Reporting Tool and Attachment F - Solicitation Clauses and Provisions. xiv.The Defense Priorities and Allocations System (DPAS) are not applicable. xv.The Request for Quote is due on or before August 17, 2016 @12:00PM EST. Quotes shall be submitted via email to Rita.Jackson@va.gov and Ken.Carter@va.gov. Any question in reference to this RFQ shall be submitted to Rita.Jackson@va.gov and Ken.Carter@va.gov and are due no later than August 15, 2016 @12:00PM EST. xvi.POCs regarding this RFQ are Rita.Jackson@va.gov and Ken.Carter@va.gov See attached document: SF1449 and accompanying documents.
