VA260-16-Q-0037 Amendment Questions from Vendors A.The purpose of this amendment is to address the questions below and include the Boise VA location that was missed on page 29, B.15 VISN 20 Locations. 1.The address for Boise is: a.Boise-531 Boise Campus, 500 W Fort St, Boise, ID 83702 2.The structure is CPRR? Not CPT? a.Please provide the proposal in the form of an inclusive price for analyzer, service, reagents, controls/ calibrators, water system credit, UPS, other items needed to run the test, into a single price per test. At the VA, we call that a CPR, but it may be called CPT (cost per test) elsewhere. Please define what is included in the cost per each unit (test) for comparison. b.The CPR will be the same run across the VISN20 for the basic system. 3.Do you want each site to have their own specific automation add on or total costs for all locations where everyone has the same price? a.Each site with their own specific automation add on. b.An added surcharge for that specific configuration for each VA location if needed. 4.Is there any HCV (Hepatitis C) volume? Below is the HCV volume. Any tests not included in this solicitation will be negotiated separately with a modification to the contract. 5.Nothing else follows.
