- Past Performance Questionnaire Combined synopsis/solicitation This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation for this requirement; quotes are being requested and a written solicitation will not be issued. This combined synopsis/solicitation HQ0034-16-T-0133 (hereby referred to as “ Solicitation ” ), for Motion Picture and Video Production Commercial Item Services, issued as a firm-fixed-price request for quotation (RFQ), with the solicitation and its incorporated provisions and clauses in effect through Federal Acquisition Circular 2005-88 and DFARS Change Notice 20160607. Submission of written (emailed) quotes must be in accordance with FAR 52.212-1, Instructions to Offerors, Commercial Items. Verbal offers will not be accepted. This acquisition is a 100 percent Women Owned Small Business (WOSB) set-aside in accordance with Section 8(m) of the Small Business Act (15 U.S.C. 637(m)) - Women-Owned Small Business Program. The associated North American Industrial Classification System (NAICS) code 512110, with a small business size standard of $32.5M annual gross revenue. All interested vendors shall provide quotes in accordance with the Statement of Work (SOW) and the appended Pricing Template (Appendix 1). The Government intends to evaluate quotes and award a firm fixed price contract without discussions unless it is determined to be in the best interest of the Government. The anticipated period of performance of this contract incorporates one (1) 12-month base period and two (2) 12-month option periods. The period of performance for the base period is September 26, 2016 – September 25, 2017 and each subsequent, 12-month option period may commence thereafter as required. Mandatory Registrations: To be eligible to receive an award resulting from this RFQ, the apparent successful quoter must be registered in the System for Award Management (SAM) and registrations must be current and contain no exclusions. Additionally, a WOSB concern eligible under the WOSB Program must have submitted documents verifying its eligibility at the time of initial offer to the WOSB Program Repository. The contract shall not be awarded until all required documents are received. If your Representations and Certifications are not current, go to https://www.sam.gov/ to register and update your account prior to the closing date of this RFQ. There will be no exceptions.
