Description

STATEMENT OF WORK Background: The listed line items are needed for the Phoenix, VAMC, new building activation. Equipment listed is required to complete installation of Dental Oral Surgery Suites. Rooms are designed to meet the needs of Oral Maxilla Facial dental care. SCOPE OF WORK: The contractor shall provide all line items to Phoenix VAMC. Contractor will supply all items and quantities listed in B.4 Price/Cost Schedule and Item Information. Description of items requested: QTY: 01 Parent System: ITEM#366-5720 OPMI pico Essential (ceiling mounted), Carl Zeiss brand name or equal (example below), Leica brand name or equal also acceptable. Package Includes: "(Quantity 1)**5 Magnification Step Microscope Body "(Quantity 1) 0-180 Binoculars "(Quantity 1) 300mm Fine Focus Objective Lens "(Quantity 2) 12.5x Eyepcs "(Quantity 1) Adapter Ring "(Quantity 1) Vision Guard/Lens Protector "(Quantity 1) Blue Dust Cover " (Quantity 1) LED Lighting System Component: ITEM#366-5951 Ceiling Mount Plate Mount plate is required for structural plans of new building and mounting specifications/cut sheets. Description of what equipment being requested is: The OPMI pico Essential dental microscope enables you to visualize high-contrast, true-colors images, improving the quality of the examination and treatment. HOW IT FUNCTIONS/OPERATES: Plug in the cable, switch the power on, and ready to operate. Microscope only- not integrated option. Yes "Who performs servicing on this machine? oThere are limited warranties included; this would be sent to Bio-Med for any repairs. DELIVERY: The contractor will deliver all items and quantities per B.5 Delivery Schedule. All items will ship to Phoenix VAMC address listed under delivery schedule, line item 1. Delivery and Installation will be made between December 1st -16th 2016, during normal business hours, excluding federal holidays. Please schedule delivery/installation with POC. VA shall not pay for freight charges exceeding $250.00, Per VAAR 847.303-1 F.O.B. origin. A copy of a packing slip will be furnished at the time the order is delivered. Installation: The contractor will coordinate with the POC to have celling mounted microscope installed and complete any user training. Installation will be complete 7 days after starting installation.
