- Please note that this is NOT a request for quotes or proposals, the Government is seeking information for market research purposes only. The Department of Veterans Affairs, VA James J. Peters VA medical center is currently conducting a market survey for qualified firms capable of providing "A comprehensive Pest Control Program for James J. Peters VA Medical Center" The vendor will be responsible for, but not limited to: A comprehensive Pest Control Program for the James J Peters VA Medical Center and associated CBOC's. This is for a base year starting October 1,2016 -Sept30 2017 with 4 option years. SERVICE APPROACH Vendor Pest Elimination provides to the VA a comprehensive service approach which includes: y Proactive Solutions: Using proprietary protocols supported by science and an outside-in approach, Vendor helps to achieve elimination and prevention of covered pests and protect our customers' facilities. y Integrated Methods: Highly-trained and professional Service Specialists provide VA with services which may include: inspection, pest monitoring, sanitation and structural recommendations, mechanical trapping, and biological and targeted product applications. SANITATION AND STRUCTURAL RECOMMENDATIONS y Each regular service visit includes a sanitation and structural inspection of the covered service zones as listed below. y Vendor will inform the VA of existing sanitation or structural conditions found by Vendor which can contribute Pest activity or to an infestation of a Pest. y Findings and recommendations will be reported in Vendor's Service Reports. CUSTOMER PARTNERSHIP To maintain a pest free environment a partnership must include but not limited to the following items: y Communicates with Vendor about pest activity or concerns which occur between regular service visits. y Provides access to all areas of the facility, including locked areas. y Allows adequate time for service when no other conflicting activities may be performed. y Promptly corrects structural issues. y Consistently practices good sanitation procedures. y Prepares the facility for services per Vendor instruction. y Performs proper post-service cleaning as directed by Vendor's Service Specialist. y Does not move, destroy or alter Vendor's supplied equipment. SERVICE COMMUNICATION AND RESPONSE TIME y Vendor Customer Service: VA may contact Vendor regarding Covered Pest activity or other pest concerns which occur between regular service visits by calling a customer service number Customer Service representatives will be available 24 hours a day, seven days a week, 365 days a year. y Call Back: An Vendor representative will call VA back within one hour after VA calls Vendor Customer Service to report a pest issue. y On-site Assessment: If, at the time of the call back, it is determined that a matter needs immediate attention, then Vendor will be at customer's facility within 24 hours or at a time mutually agreed upon by customer and Vendor. y Emergency Services: Emergency services will be available 24 hours a day, seven days a week, 365 days a year. y Service Reports: Vendor will provide a written, detailed service report to customer after each service visit. Service report information includes: pest activity found at the time of service, product applications and equipment installation, if any, and preparation, sanitation or structural concerns or recommendations. Vendor's service report is recorded using an electronic documentation system which generates an on-site report. Service reports may also be available electronically on-line or via email. y Customer Logbook: Vendor maintains an on-site logbook at each VA facility. A logbook will contain the facility's service history and required safety information, including: o Copies of Vendor's Service Reports. o CD or hard copies of product labels and Material Safety Data Sheets (MSDS). o Printed materials or forms required by applicable federal, state and local regulations. o Other relevant information concerning the services provided at customer's facility. y Trend Reporting: Quarterly trend analysis for all properties will be provided upon VA's request. Reports will document program improvements, identify areas in need of improvement and provide data for proactive decision-making. y Pest Management Reviews: Vendor and VA will conduct periodic reviews of the pest management program to help ensure the facility's needs are appropriately addressed. Services SERVICE FREQUENCY Vendor will be on property daily ( Monday-Friday) or as needed will be serviced once per month with a printed service report provided after each visit. Service information will be recorded using an electronic documentation system that is used to generate onsite, easy-to-read reports. Vendor Cockroach Program Vendor provides the following services for Cockroaches ("Cockroaches" are American, German, Oriental, Australian, Turkestan, Brown, and Smoky Brown cockroaches only): y Inspect the facility to locate and record noted Cockroach activity, harborage sites, and conditions conducive to Cockroach activity. y Provide appropriate targeted product applications according to label into cracks, crevices and voids during regular service visits. Vendor Rodent Program Vendor provides the following services for Rodents ("Rodents" are house mice, Norway rats and roof rats only): y Inspect the outside perimeter and public areas of a facility to identify signs of Rodent activity, possible entry points and conditions conducive to Rodent activity (both interior and exterior areas of the facility). y Exterior Services: Vendor will Install and maintain 75-80 Rodent bait stations around the outside perimeter of the facility. y Interior Services: Vendor will install and maintain 100-105 interior Rodent traps suitable for the facility. y Inspect, clean and date Rodent bait and trapping equipment during regular service visits. Entry Point Protection Vendor provides appropriate targeted product applications during regular service visits, as necessary, to exterior doorways and other entry points for only the following pests: Ants ("Ants" are all ants other than. carpenter, pharaoh, fire or other wood destroying insects), Cockroaches, ground beetles, springtails, silverfish, crickets, centipedes, millipedes, sowbugs, pillbugs, and earwigs. Zones to be Inspected During Regular Service Visits: y Building's outside perimeter y Dumpster area y Public areas y Restrooms y Back of house y Employee break area y HVAC/mechanical rooms y Kitchen y Offices y Storage closets y Locker Rooms y Laundry y Dietary/Food Service y Environmental Services (housekeeping, laundry, etc) y Mechanical Areas y Central Receiving and Storage y Nurses Stations y Resident/Patient areas, offices and surgery upon request Outside Perimeter Program Vendor provides targeted product applications to the immediate outside perimeter of the property for Ants, Cockroaches, ground beetles, springtails, silverfish, crickets, centipedes, millipedes, sowbugs, pillbugs, and earwigs one time per year during customer's regular service visits. Vendor Large Fly Program Vendor provides the following services for Large Flies ("Large Flies" are house, blow and bottle flies only): y Inspect the facility and identify possible sources and entry points. y Interior Services: When determined necessary, provide targeted product applications for likely areas where Large Flies may be found. Vendor will Install 88 Vendor -supplied Fly Traps. For Vendor -supplied fly traps, glue boards will be replaced monthly. Light bulbs will be replaced annually.Vendor will ensure proper recycling of bulbs. y Exterior Services: Provide product targeted product applications in dumpster area and facility entry points. Vendor Ant Program Where heavy or persistent Ant activity occurs and requires more concentrated attention than Vendor's Entry Point Protection or Outside Perimeter Programs, Vendor provides additional treatment services for Ants. Treatment solutions will be developed based upon Ant species found. Vendor provides the following services for Ants ("Ants" are all ants other than carpenter, pharaoh, fire or other wood destroying insects): y Visually inspect both the inside and outside of the facility to determine: Ant species, nest locations, Ant trails and other signs of Ant activity. y Develop and provide targeted product applications for inside and outside the facility based on Ecolab's inspection findings. y Provide recommendations to the customer to correct structural and sanitation conditions conducive to existing Ant activity. Vendor Bird Service (As-needed per survey) Vendor provides integrated bird management programs to utilize various deterrents including netting, destabilizing coils, rods, posts, wires, trapping and other non-lethal deterrent methods. If customer requests bird services, the scope of those services and the applicable fees will be more particularly set forth in a separate services agreement to be signed by the parties before services are rendered. Bed Bug Service (As-needed) Vendor will offer inspection/treatment and inspection-only bed bug programs. If bed bug activity is suspected, VA may contact Vendor regarding available bed bug programs. Vendor will perform, for a fee, a visual bed bug inspection of the suspect room for signs and presence of bed bugs. If during the preliminary inspection Vendor finds one or more live bed bugs (or evidence of live bed bugs) in a room, then that room will be considered "infested." If a room is infested, Vendor will get approval from VA o treat room at a set price per occurrence. Vendor Termite Program (As-needed per survey) Vendor will provide an annual inspection and treatment program that identifies termite activity, helps eliminate the source of current infestations, and helps prevent future infestations. Treatment plans vary based on termite species found and treatment performed. Treatment options may include: y Spot Treatments: Spot product applications are used when termite activity is confined to small areas within the structure. These applications may include the use of liquid termiticides and above ground bait stations. y Barrier Treatments: Barrier treatments are used when termite activity is found in many locations within the structure. This treatment consists of establishing or re-establishing a complete barrier around the facility. y Monitor/Baiting: A monitor/baiting service will be used when there is termite activity outside the structure. This option provides a perimeter system to help reduce activity near the structure and minimize future termite attacks on the structure.
