Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The solicitation number is VA258-16-Q-0748 and is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-88 effective 06/15/16. The associated North American Industrial Classification System (NAICS) code for this procurement is 334516, Analytical Lab Inst Mfg with a small business size standard of 1000 employees. This requirement is a set aside for: service disabled veteran owned small business (SDVOSB). If there are no approved 'or equal' products received, award will be to Roche Corporation. FOB destination: New Mexico VA Health Care System, 1501 San Pedro Dr SE, Albuquerque, NM 87108-5128. Quotes, Brand Name or Equal, for the following kit, is requested and as set forth in the Request for Quotation (RFQ) VA258-16-Q-0748: Roche Corporation COAGUCHEK XS PLUS METER KIT; quantity: 6 KT Refer specifically to the Statement of Work, paragraph 4, Scope, and B.3 Price/Cost Schedule of solicitation VA258-16-Q-0748. ***Question Submission: Interested SDVOSBs shall submit all inquiries concerning the RFQ at the earliest possible time to enable the Buyer to respond. Questions SHALL be submitted via email to: Bryan.Rogers2@va.gov. Amendments to the RFQ addressing inquiries or concerns will be posted as an amendment to the RFQ on this same website. Questions not received within a reasonable time prior to close of the RFQ may not be considered. *** New Product only; NO remanufactured or "gray market" items. All items shall be covered by manufacturer's warranty. Quote SHALL be valid for a minimum of 60 calendar days after due date of quotations. This solicitation requires registration by the vendor with the System for Award Management (SAM) prior to award, pursuant to applicable regulations and guidelines.
