Description
The Department of Veterans Affairs, Fayetteville Healthcare System of the Ozarks (VHSO), intends to solicit proposals from Service Disabled Veteran Small Businesses (SDVOSB) to provide a Medical Physicist oversight of VHSO Quality Assurance activities relative to radiation safety and medical physics including all material and machine sources of ionizing radiation and MRI. The VA anticipates issuing a solicitation for this requirement on or about September 1, 2016, with a closing date for proposals on or about September 16, 2016. The solicitation will be available for download from www.fbo.gov. Interested contractors will be responsible for downloading copies of solicitation (and any subsequent amendments) from that website, and no hard copies will be made available. The solicitation will provide full information to contractors about the proposal submission process. Prospective offerors are encouraged to register their interest in the solicitation on the FBO website so that they are notified of the issuance of the solicitation and any amendments. The successful offeror will be required to make a minimum of 4 visits, quarterly visits, as well as any additional requested visits as needed. Evaluation of radiology and nuclear medicine equipment, including acceptance testing at the time of installation, routine and emergency checks as or following a major repair that would affect the image quality or have an impact on Radiation Protection including publish ACR or Joint Commission limits. Ensure VHSO facility compliance with Nuclear Regulatory Commission (NRC) regulations/NHPP and the VA system and providing documentation stating compliance. Review the condition of all Nuclear Medicine equipment at the VHSO at least once a year or more often if equipment failed quality control, was repaired, or as needed. Perform quarterly evaluations of Nuclear Medicine records and equipment and provide a written report of findings and solutions to any issues found. Perform acceptance testing of Nuclear Medicine equipment as needed. Perform quarterly performance-based audit of laboratory practices in Nuclear Medicine and provide a written report of findings and solutions to any issues found. Due to the urgent nature of this requirement, contractors will be required to demonstrate their ability to provide services within a 1-week period after contract award. Initial contract awards are expected to be on or about October 1, 2016. Contracts will have a base term of one year, with options for four additional one-year terms exercisable at the discretion of the Contracting Officer. The Government anticipates making a firm-fixed price contract for this requirement. North American Industry Classification System (NAICS) Code is 561320 and size standard is $27.5 Million and the solicitation will be issued for SDVOSB (Unrestricted). All responsible sources may submit proposals which shall be considered by the Agency. To be eligible for award, contractors must be registered in SAM (System for Award Management) database (information on registration requirements can be obtained by the Internet at https://www.sam.gov and submit their complete Annual Representations and Certifications (ORCA) via online at https://orca.bpn.gov at proposal due date/time.
