- Complete Solicitation PART 1 - Description This is a Combined Synopsis/Solicitation for Commercial Items prepared in accordance with the format in FAR Subpart 12.6 and FAR Part 13 and as supplemented with additional information included in this notice. This Request for Quotation (RFQ) announcement constitutes the only solicitation that will be issued; a quote is being requested and a written solicitation will not be issued. The government contemplates a Firm-Fixed Price Purchase Order resulting from this solicitation. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-89 dated July 14, 2016. The associated North American Industrial Classification System (NAICS) code for this procurement is 811219 - Other Electronic and Precision Equipment Repair and Maintenance; the small business size standard is $20,500,000.00. PART 2 - Supplies or Services and Prices/Costs The U.S. Food and Drug Administration (FDA) has a requirement for one (1) base year and (4) option preventative maintenance for parts and labor covering One (1) MIS, CH14-861 900 Steris Power Pack Electric Steam Boiler, Two (2) AMSCO-Century SV-148H, One (1) ST75925 Thermo Fisher, One (1) STERIS SI-120, and One (1) AMSCO-Lab 250 LV. The establishment of a comprehensive preventive maintenance service agreement with a commercial contractor for five (5) Autoclaves and one (1) electric steam boiler. Instruments are physically located at: 22201 23rd Drive SE Bothell WA 98021 -4421 Vendors would be required to provide their services at this physical address, quarterly for each contract year. Service specifications are as follows: Vendor will provide the following as applies to each instrument identified in this contract: 1. Inspection of entire autoclave/steam generator (unit) to include: - structural integrity of the instrument, -piping -electrical circuitry and connections - gaskets - valves - siteglasses - or as physically equipped. -written report reflecting the condition of each sterilizer unit 2. Cleaning of each sterilizer unit to include: - door gasket -sterilization chamber -internal drain. 3. Replace parts as needed: Parts are purchased separately 4. Assess Performance of each sterilizer unit after servicing to include: -Thermostat performance -Internal Pressure -Perform a test cycle to demonstrate performance -written report reflecting the performance of each sterilizer unit Vendors who are not registered in SAM will not be eligible for award.
