SOLICITATION NUMBER: VA247-16-Q-0933 REQUIREMENT: Therapeutic Sleep Surface Mattresses BIDS DUE: 4:00 PM, Monday, 12 September 2016 RESPOND TO: Rena Silvestri, Rena.Silvestri@VA.gov If vendor is quoting against an FSS contract, the applicable FSS contract number must be annotated in block 2 of the cover page. Vendors must complete the vendor POC information in block 17 of the cover page. Vendor must include DUNS and CAGE Code number. Vendor must complete unit and total cost on the Price/Cost Schedule on page 3 of the RFQ. This price must be FOB DESTINATION. Vendor must submit the completed RFQ to the Contracting Official listed on the cover page and shown above no later than the Bid Closing Date/Time shown above and in block 8 of the RFQ cover page. Submittal of a vendor's quote form does not constitute submittal of a valid bid. Failure to complete and return this RFQ document may cause your firm to be considered Non-Responsive.
