Description
The purpose of this modification is to clarify the questions and amendment VA249-16-Q-0810_1 that were previously submitted and extend the close date to 8am CST, Friday, September 16, 2016. Please review attached revised SOW and Price/Cost Schedule. All other terms and conditions of this solicitation remain unchanged. QUESTIONS & ANSWERS: A.Two new locations were just added: 1689 and 1980 Nonconnah Blvd. both Regulated Medical Waste (RMW) and Sharps that are not listed in the SOW. Will they be part of this new contract? ANSWER: Yes, SOW section 3 - Place of Performance: 2 new locations are added. Sharps are not included in this solicitation. B.The SOW calls for Compactors and Front Load containers which do not haul RMW waste but are used for Solid Waste and Recycle only. Currently the location is using the following container types: 1. Main Facility - TB04 (28gl reusable tub red and TY04 28gl yellow for trace chemo) 2. The Offsite are using TB11 (32gl reusable red tub and 4.3 CF Medium Box) ANSWER: Compactors and front load containers does not apply to this solicitation. The following containers shall be used: 32 Gallon Reusable Red Tubs, 28 Gallon Reusable Red Tubs, 28 Gallon Yellow for Trace Chemo and 4.3 CF Medium Boxes SOW section 4 - Schedule of Performance: compactors and front load containers is removed. C.The main facility is currently being serviced 5X per week and the offsite locations with the exception of 1689 Nonconnah facility is being serviced every 4 weeks and this new location is every week. The SOW calls for 3X week service at the main facility which we do not feel is adequate service based on the volume from the main facility. Should the service level be listed as 5X per week or 3X per week? Please advise. ANSWER: Main facility 5X per week and other locations biweekly. D.SOW calls for labor for packaging medical waste? Is this truly a requirement of the contractor as it isn't part of this contract currently? Please advise. ANSWER: No. E.Per the SOW it is a requirement that the transporter has a State permit and license number on the containers? We are asking because it is not a State or Federal requirement for vendors to have their state permit and license number on the containers. Please advise. ANSWER: Not required. F.The SOW needs to clarify that Bulk Chemo waste is not part of this bid as Bulk Chemo is considered a RCRA Hazardous Waste and would require a more detailed SOW that includes Hazardous Waste disposal. Please advise. ANSWER: Not generating in bulk, limited chemo waste G.How the pricing structure should be provided by vendors? Should vendors provide pricing by the Pound or by the Box? ANSWER: By the pound. H.The waste volumes that are currently listed in the combined synopsis/solicitation for the main facility are incorrect as we have already pointed out. What should this be? I attached the SOW to show what I submitted. Please advise. I. ANSWER: The South Clinic is going away and relocated within Noncannah (2) Buildings. J.Please clarify SOW request for open tops and compactors, the use of these containers are illegal in the State of Tennessee and compaction is against EPA regulations. ANSWER: They are not needed. K.Please provide quantity (in pounds) of medical waste produced in Memphis VAMC. ANSWER: Average monthly quantity is 7548.70 lbs.
