Description: Description of Required Services Quality Assurance Surveillance Plan OCI Certificate This is a combined synposis/solictation notice in accordance with Federal Acquisition Regulation (FAR) 12.603. This requirement is in support of the US Army Public Health Center (APHC) (Provisional) located at Aberdeen Proving Ground South - Edgewood, Maryland and is an invitation for Request for Quote (RFQ). The requirement is for a twelve-month period of performance beginning 30 September 2016 through 29 September 2017. This requirement is unrestricted with performance at the contractor's facility. The Contract Line Item Structure (CLIN) is as follows; Firm-Fixed-Price - Services in accordance with the Performance Work Statement (PWS); Quantity of '1'; Unit of Measure of 'Job'; Firm-fixed-price - Contractor Manpower Reporting; Quantity of '1'; Unit of Measure 'Each'. STARTING AT SECTION “A” BELOW** (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: A.1 BASIS FOR AWARD A.1.1. The Offeror’s proposal will be evaluated to ensure the Government receives a technically acceptable award at a fair and reasonable price. The selection for award will be based on the following equally important non-cost/price factors: Factor 1 - Technical Approach Factor 2 – Past Performance Factor 3 - Price A.1.2. The Government evaluators will not assume that the Offeror’s proposal possesses any capability or knowledge unless it is specified in the proposal. A.1.3. The Government intends to evaluate and award contract without discussions. However, the Government reserves the right to conduct discussions. A.1.4. If the above non-price factors receives an individual rating of “Unacceptable,” the Offeror’s proposal will be deemed “Unacceptable”. Only a proposal receiving an overall rating of “Acceptable” will be considered for award. A.2. EVALUATION CRITERIA Factor 1 – Technical Approach: The Offeror’s proposal will be evaluated for acceptability based on demonstrating an understanding of the PWS and of the functional and technical tasks to be performed. The Offeror’s proposal shall state how the proposed Technical Approach will yield the required results as stated in the request for proposal’s required time frame. The Offeror should discuss the methodology and flexibility that will be used in performing the requirements. Factor 2 – Past Performance: Factor 2 - Past Performance: Offeror's past performance records will be evaluated to the degree to which Offeror demonstrates the relevant and recent past performance information that pertain to the effort and services to be acquired as outlined in this solicitation requirements. Offeror's past performance history will be evaluated to the degree to which Offeror demonstrates quality performance and the most confidence in satisfying Government requirements stated in this solicitation. Factor 3 – Price: Price will be evaluated separately from other non-price factors and will be evaluated for fairness and reasonableness. The total evaluated price will be evaluated for reasonableness in terms of: a. Consistency with the requirement’s acceptable technical capabilities in the Offeror’s non-price proposal; b. Price proposal evaluation is to determine whether vendor’s price proposal is fair and reasonable, the Government will evaluate the completeness and reasonableness of the Offeror’s price proposal. c. A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer’s specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. A.3. ACCEPTABILITY RATING STANDARDS: An evaluation for technical acceptability will be made to ensure satisfactory responsiveness to the requirements as set forth in this solicitation. Each non-cost/price factor will be rated “Acceptable” or “Unacceptable” using the following rating standards: A.3.1. For Factor 1 – Technical Approach standards will be: Acceptable: Proposal clearly meets the minimum requirements of the solicitation. Unacceptable: Proposal does not clearly meet the minimum requirements of the solicitation. A.3.2. For Factor 2 – Past Performance rating standards will be: Acceptable: Based on the Offeror’s performance record, the Government has a reasonable expectation that the Offeror will successfully perform the required effort, or the Offeror’s performance record is unknown. (See note below.) Unacceptable: Based on the Offeror’s performance record, the Government does not have a reasonable expectation that the Offeror will be able to successfully perform the required effort. Note: Offeror without a record of relevant past performance or for whom information on past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the Offeror may not be evaluated favorably or unfavorably on past performance. The Offeror's past performance records shall be determined to have unknown (or 'neutral') past performance. In context of acceptability/unacceptability, a neutral rating shall be considered 'acceptable.' A.3.3. For Factor 3 - Price Price will be evaluated separately from non-price factors and past performance and will be evaluated for fairness and reasonableness. The Government will evaluate Offeror's price proposal in terms of: a. Consistency with Offeror's non-price proposal; b. Completeness, fair and reasonableness. A.4. (d) Comptroller General Examination of Record. (1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract.
