- This is not a solicitation announcement. This is a Sources Sought publication only. The purpose of this Request for Information (RFI) is solely for information and planning purposes and does not constitute a solicitation. Please do not submit a proposal in response to this RFI. This notice is to assist the VA in the determination of sources only. A solicitation is not currently available. Responses to this sources sought is not a request to be added to a prospective bidders list or to receive a copy of the solicitation. The Department of Veterans Affairs (VA), Fargo VA Health Care System, is seeking potential Contractors capable of providing 2.8 Full-Time Equivalent (FTE) Board certified/ Board eligible Inpatient Hospitalist Physician Services on site at the Fargo VA Medical Center, 2101 Elm St. N., Fargo, ND 58102. These services are solely dedicated to Veteran patients of the Fargo VA Health Care System. The Fargo VA is looking for responses from contractors regarding their capability to provide 2.8 FTE Inpatient Hospitalist Physician Services. 2.8 FTE is defined as 113 hours per week; 5881 hours per year. Hours of operation at the Fargo VAHCS for Hospitalist service providers are 24 hours per day, seven days per week. They are scheduled to be onsite during this time. There are two shifts that encompass this period: 7:00 AM to 7:00 PM and 7:00 PM to 7:00 AM. Services to be provided include: Contractor's physician(s) (As appropriate, within scope of practice/privileging, and capabilities of the Fargo VAHCS) shall be responsible for providing Hospitalist service care, including, but not limited to: "Evaluation, Emergent Treatement and Management: Employment of the principles of inpatient care for life/limb threats, resuscitation and stabilization, triage, diagnosis and disposition that may occur on an inpatient medical ward, ICU, and long-term care facility: oEvaluation, treatment and management of minor wound care, respiratory illness, gastrointestinal illness, burns, musculoskeletal trauma, dermatological illness, ENT, eye and urological problems. oEvaluation, treatment and management of minor procedures such as local infiltration anesthesia, incision and drainage, simple laceration repair, nail trephination, electro-coagulation, nasal cautery, gastric lavage, bladder catheterization, peripheral venous line insertion, and spinal immobilization. oEvaluation, treatment and management of abdominal and gastrointestinal disorders (including trauma) of the esophagus, stomach, small bowel and colon rectum and anus, liver and biliary tree and pancreas. oEvaluation, treatment and management of cardiovascular disorders (including trauma) involving cardiac failure, differential diagnosis of chest pain, cardiac structural disorders, cardiac rhythm and conduction defects, pericardial disorders, disease of peripheral arteries and veins, shock, and cutaneous disorders. oEvaluation, treatment and management of end of life care (Advanced directives/ Palliative care) oEvaluation, treatment and management of emergent disorders caused by antigens, organisms and other foreign substances such as reactions of hypersensitivity; reactions from venoms, bites and stings; reactions caused by infectious agents; disorders due to chemical, drug and physical agents; and disorders associated with the environment to include barotraumas, near drowning, electrical injury, hypothermia and radiation injury. oEvaluation, treatment and management of emergent disorders of the hematopoietic system such as anemia, coagulopathy and management of acute neoplastic disease complication. oEvaluation, treatment and management of emergent disorders of endocrine, metabolic and nutritional natures relating to acid-base disturbances, adrenal, parathyroid and thyroid disturbances. oEvaluation, emergent treatment and management of emergent disorders of the head and neck (including trauma) involving the ears, nose, oral cavity, larynx/trachea, face and vestibular system. oEvaluation, treatment and management of post-emergent disorders (including trauma) of the eye involving the lids and lachrymal apparatus conjunctiva, cornea, sclera, internal aspects of the globe and orbit. oEvaluation, treatment and management of post-emergent disorders (including trauma) of the musculoskeletal system involving shoulder girdle, upper extremity and hand, lower extremity and foot, thorax and vertebrae and arthopathies. oEvaluation, treatment and management of post-emergent disorders (including trauma) of the nervous system including cerebral edema, coma, cranial nerve disease, cerebro-vascular disease and infection. oEvaluation, treatment and management of post-emergent respiratory disorders including pulmonary, infection, trauma, neoplasia, metabolic and complications of cardiovascular disease. oInitial evaluation, treatment and management of renal and urologic disorders including acute/chronic renal failure, infections, obstructive uropathy and hematuria and trauma. oEvaluation, treatment and management of OB/GYN disorders such as post-emergent trauma, infection and pregnancy (ectopic and intra-uterine). oSuture minor lacerations. "Stabilization and transfer: Patients suffering traumatic injuries where immediate treatment and release is not medically indicated and when clinical level of care is not available at the Fargo VAHCS, the contract physician(s) shall provide assistance as needed to the emergency department to provide initial stabilization and readying of such patients for transport to higher levels of care. "Duty: Contractor physician(s) shall be present ontime for any scheduled tours as documented by physical presence in the appropriate location of the Fargo VAHCS, at the scheduled start time. "Inpatient Admissions: Contractor physician(s) shall review all admissions to inpatient hospital care recommended by Mid-level (Physician Assistant or Nurse Practitioner) providers as needed. Every admission to inpatient care shall have a person-to-person hand-off/hand-over from the admitting provider to a responsible member of the admitting team. "Consultation and Referral Responsibilities: Contractor physician(s) shall provide consultation with, and instruction to, referring physicians regarding appropriate indications for procedures so that the most expeditious and clinically appropriate work-up can be done. Contractor physician(s) shall determine the appropriate course of treatment and communicate in person or by phone with the referring clinicians. oContractor physician(s) shall initiate appropriate social work referrals for all identified homeless veterans and for patients who do not have primary care providers, but who appear regularly in the Fargo VAHCS. "Medications: Contractor physician(s) shall follow all established medication policies and procedures. No sample medications shall be provided to patients. "Discharge education: Contractor physician(s) shall provide discharge education, medical reconciliation, and follow up instructions that are coordinated with the next care setting for all Inpatient Hospitalist patients. Additional Information: The VA seeks to gain knowledge of potential qualified sources and their size classification (HUBZone, 8(a), small, small disadvantaged or service disabled veteran owned small business) relative to NAICS Code 621111, Offices of Physicians (except Mental Health Specialists); size standard of $11 Million. Responses to this Sources Sought will be used by the Government to make appropriate acquisition decisions and developing a potential procurement strategy. After review of responses to this sources sought synopsis, a solicitation announcement may be published in the FedBizOpps website. Responses to this sources sought synopsis are not considered adequate responses to the solicitation announcement.
