- This is a combined synopsis/solicitation for a commercial item prepared in accordance with all parts of and format in Subpart 12.6, as supplemented with additional information included in this notice in conjunction with procedures in FAR Part 13. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-89. Only emailed requests received directly from the requester are acceptable. Email quotes to sam.demuzio@va.gov no later than the required due date of Tuesday, September 20, 2016 on or before 12:00 PM (Noon) Eastern Time. Receipt must be acknowledged on my desktop computer to be recognized as received. This is the only acceptable method for delivery of quotes. Products shall be supplied in accordance with the specifications, terms and conditions contained herein. In addition, the following FAR and VAAR clauses will also apply. The Buffalo VAMC is seeking to purchase four (4) Reliance ophthalmic exam chairs, four(4) Haag-Streit Tonometers, four(4) Haag-Streit Slit Lamps, four (4) Reichert 11625 Ultramatic Phoroptor s, and four (4) Marco Ophthalmic 1290 equipment stands that meet the clinical, functional, and performance specifications specified in this Statement of Work (SOW). The Buffalo VAMC shall only consider new, latest-generation products. Refurbished or remanufactured will not meet the needs of the Buffalo VAMC. 1.TONOMETER The proposed ophthalmic tonometer shall be a Haag-Streit USA AT900 BQ Tonometer. The proposed tonometer shall be compatible with the Haag-Streit BQ900 slit lamp. The proposed tonometer shall feature reusable tonometer prisms. 2.EXAM CHAIR The proposed ophthalmic exam chair shall be a Reliance Medical Products, Inc 7000 Exam Chair. The proposed exam chair will allow the Doctor to control the chair from both sides. The proposed exam chair will feature an adjustable headrest. The proposed exam chair will provide a corded footswitch for hands-free operation of the chair base and top. The proposed exam chair will have the ability to swivel 330 degrees. 3. PHOROPTOR The proposed refractor shall be a Reichert 11625 Ultramatic Phoroptor. The proposed refractor shall have a limited lifetime warranty. The proposed refractor shall have the following technical specifications: i.Sphere: -19.00D to +16.75D ii.P.D. Adjustment: 48mm to 75mm iii.Cylinder Power: 0.00D to 6.00D iv.Cylinder Axis: 360deg (Double 0deg-180deg scale) v.Cross Cylinder: +/- 0.25 vi.Prism: 0-20 vii.Vertex Distance: 13.75mm viii.Reading Distance: 5" to 28" ix.Auxiliary Dial: 10 lenses plus 2 open apertures 4.EQUIPMENT STAND The proposed equipment stand shall be a Marco Ophthalmic 1290 Encore. The proposed equipment stand shall provide at least three rechargeable wells. The proposed equipment stand shall provide at least three 115V electrical receptacles. The proposed equipment stand should have a three-way locking counterbalanced refractor arm. The proposed equipment stand should have an attached overhead patient light. 5. SLIT LAMP The proposed slit lamp shall be a Haag-Streit model BQ 900 LED. The proposed slit lamp shall provide an overall magnification range from 6.3x to 40x. The proposed slit lamp shall have the following technical specifications /capabilities with respect to its microscope: "12.5x microscope eyepiece magnification. "Microscope stereo angle of 13 degrees "Microscope shall have +7 to -7 diopters and an inter-pupillary distance range of 52 - 78 mm. The proposed slit lamp shall have an LED light source for slit and background illumination. The proposed slit lamp shall have a ±90 degree slit image rotation range and have the following filters: Blue, red-free (green), and grey (10%). FOB Point: Destination Delivery shall be to: Buffalo VA Medical Center 3495 Bailey Avenue Buffalo, NY 14215 Government Requested Delivery: 60 days or less. Offers must be valid for 60 days after close of solicitation Offerors shall be registered in the System for Award Management (SAM) prior to award. Registration information is located at www.sam.gov. Alternate system submission: Offerors submitting alternate items must provide technical information sufficient for a lay person to determine acceptability. Information will not be paid for by the government nor returned to the supplying party. The information will be used to determine product acceptability. Lack of sufficient information for our staff to determine product acceptability will result in a negative determination. Any quote will then be found non-responsiveness to the solicitation and the quote will not be considered. New Equipment ONLY; NO remanufactured, used/refurbished or "gray market" items. All items must be covered by the manufacturer's warranty. No product in development shall be considered. Authorized dealers: Experienced firms only who are authorized dealers will be considered in addition to the original equipment manufacturer. A letter from the manufacturer stating your company is an authorized dealer for the line items must be in possession by the due date and be available for submission, if requested.
