Description
RFQ3 for Fort Hood Texas LIMITED SOURCE JUSTIFICATION 1. Defense Logistics Agency (DLA) Troop Support, Medical Supply Chain, Electronic Catalog (ECAT) Division, is the contracting activity. 2. The action submitted for approval is the acquisition of a brand name product under Electronic Catalog (ECAT) contract or other competing vendors. 3. The Carl R. Darnall Army Medical Center, Fort Hood, Texas has the following requirement for the Berchtold Operon R D850 surgical table which is a logical follow-on to replace equipment at the end of its life cycle, to facilitate the best outcomes for surgery patients, and to support the bariatric, gynecological, ear, nose and throat, general and urology surgical teams within the center. Item: Operon R D850 Surgical Table (includes leg section OZ 1705975) MFG Part Number: Berchtold OT 1702075 Quantity: 6 Each 4. This action is being conducted under the Multiple Award Schedule Program. The authority waiving the competition requirement is FAR 8.405-6 (b)(1), DFARS 208.405-1, Order Placement - only one manufacturing source meets the capability of providing the supplies or services required under FAR 16.505(b)(2)(ii). The supplies or services ordered are highly specialized and unique to Berchtold. 5. The center is replacing end of life cycle surgical tables as a logical follow-on purchase for its surgical teams. To ensure standardization and enhance patient outcomes, existing table attachments, leg and shoulder extenders, need to be compatible with the new surgery tables. These new surgical tables will not require purchase of separate table attachments. There will be a work stoppage for that provider which will disrupt patient care. Minimum clinical performance requirements: - Slide top surgical table with power that supports 1,000 lbs of articulation using slide, trend, reverse trend, tilt, back or leg section. - Table must have 100% carbon fiber table top - Facilitates c-arm and x-ray imaging - Must have an integrated x- - ray imaging window underneath table must be at least 18" and support oblique, AP and lateral c-arm imaging. Must not utilize "buckey boards" or x-ray risers. - Supports a low table height of 22.6" at lowest point and 46.4" at its highest point - Supports a 1,000lbs weight capacity that must not change when the table is slid towards the head. - Must support in-house attachments - Should have hand pendant powered kidney elevator and full capacity backup hand pendant. Table should have two hand pendants. - Should have a full manual backup sytem which allows for basic table positions in event of power failure or outages. - Should have a service software port to facilitate service diagnostics and software upgrades. - Requires FDA certification 6. DLA Medical, ECAT Division has already determined the price of the Operon surgical table to be fair and reasonable based on current vendor catalog pricing and minimum Government discounts per an ECAT contract. ECAT contract vendors will be issued a Request for Quotation (RFQ) prior to the placement of the delivery order in an attempt to obtain better pricing than was previously negotiated. This acquisition represents the best value to the Government consistent with FAR 8.404(d) and will result in the lowest overall cost alternative to meet the Government's needs. 7. Market research was conducted by the center equipment procurement staff. Vendors were evaluated using the internet and vendor catalogs that were on Government contracts. Evaluations were based on costs, Government pricing, ability to use center surgical table attachments, FDA certification and, minimum surgical team requirements. All evaluated products from Government vendors had equipment with one year warranty service and met FDA certification. Operon D850 was the preferred table on the basis it has compatible equipment with CRDAMC operating room equipment, would not require purchases of additional attachments and its cost. Without the procurement of these items, CRDAMC has reached its end of life cycle for surgical tables. CRDAMC also has a high likelihood of having patient care curtailed, losing revenue, having a negative impact to patients and surgical operation teams and/or causing surgeries to be outsourced to another facility. Additional market research was performed on August 12, 2016 by searching this particular manufacturer's part number and item description using past DoD purchases and the DLA Medical Database. There were no additional sources found that meet the CRDAMC requirements for this surgical table. 8. There are no additional facts supporting this justification. 9. ECAT vendors have the opportunity to add products that fall within the scope of the ECAT contract at any time. DLA Troop Support will continue to attempt to locate other acceptable sources for the ECAT program for future requirements as they arise; however, any new items would need to meet the U.S. Army and DoD requirements and undergo testing and evaluation.
