Museum Layout v2 Combined Solicitation/Synopsis Solicitation Number: HT0011-16-R-01695 Classification Code:99 Set Aside: Full and Open NAICS Code: 484210 CONTRACTING OFFICE ADDRESS: Defense Health Agency (DHA) 7700 Arlington Blvd Falls Church VA, 22042 This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a separate written solicitation will not be issued. ITEM DESCRIPTION: **Please NOTE this requirement will require a site visit required prior to submission. ** (POC) Barton Carlson at barton.g.carlson.civ@mail.mil (301)319-3319 1) Retrieve 10,080 aluminum replacement trays and 2,768 Burroughs replacement shelves from government warehouse and deliver to Building 178; 2) Replace 10,080 defective, previously-installed High-Density Polyethylene (HDPE) trays with equivalent aluminum replacement trays (to be provided by the government) with removal and replacement of populated glass histology slides, 3) Install ethafoam sheet (to be provided by the government) in bottom of tray beneath slides and 4) Procure and install rubber bumpers at the ends of brackets on which the trays rest; 5) Replace 2,768 defective, previously-installed Boroughs P2 shelves with equivalent boroughs P3 shelves (to be provided by the government) with removal and replacement of populated objects and other items and affix new bar-coded location labels to the replacement shelves and install associated brackets. **Please NOTE this requirement will require a site visit required prior to submission. ** (POC) Barton Carlson at barton.g.carlson.civ@mail.mil (703)319-3319 Note: Vendors are to submit delivery time with their quote of no later than four weeks after receipt of order.
