- Scope of Work Patient Lift Preventive Maintenance & Testing Service Contract Providence VAMC Providence, RI 5/2/2016 The contractor shall provide all equipment, labor, materials, any apparatus or tools, transportation, supervision, permits and disposal necessary for performing annual preventive maintenance and testing of seventy-two (72) overhead lift systems, seven (7) portable patient lifts and twelve (12) patient stand assists at the Providence VA Medical Center, 830 Chalkstone Avenue, Providence, RI 02908. The contractor shall also be responsible for any installations, relocations or removals of the overhead lift systems as required by the hospital. A. Preventive Maintenance - Ceiling Mounted 1.The contractor will provide annual preventive maintenance to a maximum of 72 overhead lift systems. The lift system consists of the overhead lift with accompanied components or accessories, the overhead lift charger, and the rail system (either traverse or fixed rail). The rail system includes any rails that are intended for use by that overhead lift system. The rail system is not defined only by breaks or gaps in the rails. 2.The stationary lifts are all Liko Likorall 250ES overhead lift systems. 3.The lifts are located in the Emergency Department, Dialysis Department, Nuclear Medicine Department, Endoscopy Department, Intensive Care Unit (ICU), Physical Therapy Department, Occupational Therapy Department, Post Anesthesia Care Unit (PACU), examination rooms, patient rooms and procedure rooms. 4.The preventive maintenance check list will include but not be limited to the following list of tasks: "Check for visible damage to the lift surfaces, finish, etc. "Check the batteries for proper voltage per battery cell. "Check that the rail system end stops are positioned properly and are tight. "Check that all of the bolts are tightened on the entire overhead lift system. "Check the movement of the lift carriage along the entire distance of the track. "Check the movement of the traversing rail along the entire distance of the fixed rails. (traversing lift systems only) "Check the emergency stop cord function. "Check the hand control functions. "Check the emergency lowering/raising (electrical) functions. "Check the function of the squeeze protection limit switch. "Check the entire lift strap for fraying, excessive wear or other visible damage. "Inspect the sling bar bearings and safety latches. "Check the charger functions to include: wall charging station, In-Rail Charging or MultiStation Charging capacities. "Check the type/model identification decal. 5.The contractor will complete an annual Preventive Maintenance check sheet provided by the PVAMC for each lift based on the manufacturers' recommendations and the VA's requirements. 6.A list of repairs required as a result of the inspection will be provided with a cost proposal for the repair. 7.The exact number of lifts may change during the life of the contract so the contractor will provide a "cost per lift" when the minimum number of lifts is fifty (50) and the maximum is seventy-two (72). B. Preventive Maintenance - Portable Lift 1.The contractor will provide annual preventive maintenance to seven (7) portable patient lifts. 2.The portable lifts are all Arjo - Maxi Move. 3.The lifts are located throughout the hospital and can be brought to a central location for the preventive maintenance. 4.The contractor will complete an annual Preventive Maintenance on a contractor provided check sheet for each lift based on the manufacturers' recommendations and the VA's requirements. 5.A list of repairs required as a result of the inspection will be provided with a cost proposal for the repair. C. Preventive Maintenance - Patient Stand Assist Lift 1.The contractor will provide annual preventive maintenance to twelve (12) patient stand assist lifts. 2.The patient stand assist lifts are all are all Arjo - Sara Lift. 3.The lifts are located throughout the hospital and can be brought to a central location for the preventive maintenance. 4.The contractor will complete an annual Preventive Maintenance on a contractor provided check sheet for each lift based on the manufacturers' recommendations and the VA's requirements. 5.A list of repairs required as a result of the inspection will be provided with a cost proposal for the repair. D. Battery Replacement 1.The contractor will provide, install and warranty the replacement batteries for each lift every 3 years continuing with the PVAMC current Preventive Maintenance Program already in place. 2.The contractor will be responsible for the proper disposal of all replaced batteries and associated rubbish per current federal, state and local requirements. 3.The contractor will also be responsible for proper storage of replacement and replaced batteries while the process is undergone. 4.The batteries must be of equal or greater quality than the batteries being replaced and also adhere to the manufacturer's recommendations for battery replacement. E. Installation, Relocation and Removal 1.The contractor will be responsible for installing, relocating or removing overhead lift systems as required by the PVAMC. 2.The contractor will provide and submit cost estimates and any structural or mechanical information or data as needed to install new, relocate or remove existing stationary lifts. 3.The contractor will be issued a Task Order for required installation, relocation or removal based on an approved contractor proposal. 4.The contractor must adhere to all current state, local and VA required building codes when placing an overhead lift system in a new location. It is the contractor's responsibility to make themselves familiar with these requirements. F. Load Testing Contractor will provide a load test on each lift to the units' maximum load rated capacity and provide documentation of the success/failure of the test. The maximum load ratings are between 450lbs and up to 1100lbs. G. Emergency Repair Service 1.Contractor will provide an hourly rate for repair service done Monday through Friday from 7:00 am to 4:30 pm. 2.Contractor will guarantee emergency repair service will begin within 24 hours of notification of repair being needed by the facility. H. Infection Control Contractor shall conform to the infection control measures as specified in the attached Construction Specification for Infection Control. I. Contract Security 1.The C& A requirements do not apply and a Security Accreditation Package is not required. 2.VA sensitive information will be protected because the contractor will not have access to the in-house computer system and will be escorted by VA personnel during the PM's and/or repair service. J. Work Hours Work hours will be from 7:00 am to 4:30 pm Monday to Friday. K. Duration This service contract will be for a base year and four (4) option years beginning in October 2016. L. Pricing 1.The annual overhead lift system preventive maintenance & testing per lift cost $_____________. 2.The annual portable lift preventive maintenance & testing per lift cost $________________. 3.The annual patient stand assist lift preventive maintenance & testing per lift cost $_____________. 4.The total annual preventive maintenance & testing cost based on seventy two (72) overhead lifts, seven (7) portable lifts and twelve (12) patient stand assist lifts is $_____________. 5.The hourly rate for weekday 7:00am to 4:30pm repair service calls on the overhead, portable and patient assist lift systems $____________.
