Statement Of Objective Statement Of Objective Request for Information (RFI) - HT0011-16-RFI-0015 THIS IS A REQUEST FOR INFORMATION (RFI) TO HELP THE GOVERNMENT TO BETTER DEFINE ITS REQUIREMENT (SMALL APPLIANCES /REPAIR MAINTENANCE SERVICES) AND IDENTIFY POTENTIAL SOURCES THAT COULD FULFILL THE GOVERNMENT'S REQUIREMENT. RELEASE DATE: 11/2/2016 CLOSING DATE: 11/28/2016 1. Purpose of this Request for Information (RFI) This RFI is being issued by the Defense Health Agency (DHA), Contract Operation Division Falls Church (COD-FC) to conduct Market Research on Industry's Capabilities in providing the DHA with nationwide (CONUS) services for repairing, warranting and upgrading small appliances. 2. Scope: a. The Government is interested in finding a contractor willing to assume the operational responsibility of the residential refrigerators and microwaves that currently existing within all of the DHA facilities. The "operational responsibilities" will be as follows: i. Provide Preventative Maintenance services and repairs. ii. Provide new commercial grade appliances when the existing appliances fail or repair costs out way the cost of replacement. iii. Provide a buy list of new commercial grade refrigerators and microwaves to the Government. iv. Upon selection of new commercial grade equipment the contractor will purchase, install, perform PM services, provide repair services and enact warranty claims as directed by the Government. v. Provide an upgrade timeline based on new equipment projected lifespan to included automatic replacement. 3. How to Respond This is an RFI ONLY; it is not a request for competitive proposals. It is issued solely for Information and Planning Purposes - It does not constitute a Solicitation nor does it restrict the Government as to the ultimate Acquisition Approach. In accordance with Federal Acquisition Regulation (FAR) 15.201, responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Should your firm desire to submit a capability statement, it should demonstrate the Offeror's experience and expertise to provide the services identified above within the time frame required and without substantial duplication of cost to the Government. The statement shall include business size; ability to self-perform work; layers of subcontracting; and any other information indicating a legitimate ability to meet these specific requirements. Responses should not exceed 25 pages (including responses to questions and capabilities statement) in length. Responders are advised that the U.S. Government will not pay for any information or administrative costs incurred in response to this RFI. All costs associated with responding to this RFI will be solely at the interested vendor's expense. Not responding to this RFI does not preclude participation in any future Request for Proposal (RFP), if any is issued. The formal closing date for this RFI and submission of Responses is Wednesday, November 28, 2016 at 10:00 AM EST. All responses shall be submitted electronically, with Subject "Company Name, HT0011-16-RFI-0015 -Small Appliances Services/Repair Maintenance" to the following e-mail to Contracting Officer, Alim Koroma, alim.a.koroma.civ@mail.mil and Contract Specialist, Xiomarys Vargas, xiomarys.a.vargas.civ@maill.mill Statement of Objectives Small Appliance Service/Repair Maintenance Requirement I. BACKGROUND The Defense Health Agency (DHA) is a joint, integrated Combat Support Agency that enables the Army, Navy, and Air Force medical services to provide a medically ready force and ready medical force to Combatant Commands in both peacetime and wartime. The DHA supports the delivery of integrated affordable and high quality health services to Military Health System (MHS) beneficiaries and is responsible for driving greater integration of clinical and business processes across the MHS. II. OBJECTIVE OR SCOPE OF EFFORT The Defense Health Agency (DHA) is interested in obtaining support to provide preventative maintenance services for small appliances; i.e. refrigerators and microwaves. The DHA exists in multiple locations throughout the United States with the largest concentration being in the National Capital Region (NCR). Other locations include Colorado, Texas, Michigan, California and Alaska. The purpose of this SOO is to obtain a Preventative Maintenance (PM) vendor to support the DHA in its multiple locations. Offerors are requested to submit a Request For Information (RFI) response for the services to support this initiative. a) The overall Program Requirements are as follows: i. To provide repair services for refrigerators and microwaves that are identified in attachment 1. ii. To provide replacement consumable parts such as water filters to item identified in attachment 1. iii. To provide replacement of units upon failure or when repair costs exceed replacement costs for the items identified in attachment 1. iv. To provide a leasing program for replacement units. (Vendor owned equipment). v. An upgrade program based on life cycle of available parts, discontinuation of models or advancement of technology. The long term goal of this Preventative Maintenance is to have the vendor provide all small appliances. As part of supplying and installing the appliances the vendor will repair and/or replace the units upon malfunction or failure. III. TASKS/DELIVERABLES a) Provide Preventative Maintenance services for all small appliances throughout the DHA organization. b) Repair services for refrigerators and microwaves. c) Replacement of consumable parts. d) Provide replacement units upon failure. e) Provide a leasing program for replacement of units. f) Provide an upgrade program based on equipment life cycle. IV. SUBMISSION OF RESPONSES All interested vendors shall submit their capabilities statement along with responses to the questions identified in Section V electronically to the Contracting Officer, Alim Koroma, alim.a.koroma.civ@mail.mil and Contract Specialist, Xiomarys Vargas, xiomarys.a.vargas.civ@mail.mil. V. QUESTIONS a) Has your company provide this service or similar service to the Federal Government in the past? If so please provide the Agency Name and Point Of Contact; Contract Number and Contract Value. b) Has your company entered into a lease type program where you have purchased and supplied equipment to the Federal Government in the past? If yes, please provide details. c) Do you have the qualified personnel currently on staff to repair appliances and diagnosis trouble issues? If yes, how many personnel and at what location(s). d) Does your company currently exist is the areas stated in this Statement Of Objective? If not how will you provide service? Please provide your office locations? e) Do you currently have personnel that can obtain a Federal Government access badge? If yes, how many. f) Do you company have a Federal Supply Schedule? If yes, please provide the schedule name and your schedule contract number. g) After reviewing the Statement Of Objective and the items identified in attachment 1, how much efforts would your company estimate it would require to meet this requirement?
