- Pricing Workbook for N62645-17-R-0003 Section J Attachment 2 Past Performance Reference Sheet Section J Attachment 1 Past Performance Information Sheet Solicitation This solicitation is for various medical services that include the labor bands of Physician, Allied Health, Advanced Practice Nurse, Nursing, Technologist, Technician, and Assistant at Military Treatment Facilities in the Southeastern Region of the United States to include: Naval Hospital Jacksonville, FL; Naval Health Clinic Corpus Christi, TX; Naval Hospital Pensacola, FL; Naval Hospital Camp Lejeune, NC; Naval Hospital Beaufort, SC; Naval Hospital Cherry Point, NC; Naval Health Clinic Charleston, SC; Navy Medicine Operational Training Center Pensacola, FL, and any associated branch clinics. Supplementation of personnel at any DoD or Coast Guard Military Treatment Facility (MTF) who have been granted authority under §10 USC 1091 and whose personnel are located within the states of North Carolina, South Carolina, Texas, Florida, Louisiana, Kansas, Indiana, Tennessee, Mississippi, Alabama, Georgia, and Virginia shall be considered within the scope of the solicitation. This is a competitive procurement for health care workers that will result in a Multiple Award Task Order (MATO), Indefinite Delivery Indefinite Quantity (IDIQ), personal services type contract. Services will be required beginning on 01 October 2017, for up to 60 months. The North American Industry Classification System (NAICS) code for this requirement is 622110. This requirement is a 100% small business set-aside. The Small Business Administration (SBA) size standard for the NAICS Code 622110 is $38.5M. This solicitation is by electronic means only. No hard copies will be mailed to offerors.
