Contract Title. Purchase of The Centurion Vision System 3. Background. The Raymond G Murphy specializes in the surgical treatment of cataract surgery and other ophthalmologic issues. The NMVAHCS has hired additional providers to handle the increasing number of veteran patients and are in the process of opening additional OR suites to care for our veterans. 4. Scope. The NMVAHCS is requesting 2 Centurion Vision Systems to replace our current models accommodate increasing case load and is necessary to provide safe standard of care to our veteran population. 5. Specific Tasks. This purchase will equip the operating room with the leading technology to perform cataract surgery. The CENTURION ® Vision System is the only intelligent phaco platform that dynamically optimizes every moment of the cataract removal procedure. With a growing collection of innovations designed to provide improved patient outcomes, the CENTURION ® Vision System sets a new standard in phacoemulsification technology. The CENTURION ® Vision System is designed to provide: A new standard in anterior chamber stability. Excellent emulsification efficiency. An upgradeable, future-oriented OR. Enhanced ergonomics for surgeon comfort. Surgical technology for a best-in-class cataract suite. 5.1 Tasks 1 - Enterprise Management Controls. 5.1.1 Subtask 1 - Integration Management Control Planning. Performance Monitoring: Biomedical engineering staff will judge the accuracy and quality of the devices procured within the 1 year warranty. If the products are not up to standards, then a repair, exchange, or credit on behalf of the vendor will be required Routine inspections of purchased items will be checked for sterility, packaging and outdates quarterly, Documentation of Alcon Company Representative performance will be done yearly.. 7. Security Requirements: The C&A requirements do not apply, and that a Security Accreditation Package is not required. 8. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). N/A 9. Other Pertinent Information or Special Considerations. This is a request for other than full and open competition for the procurement of 2 Centurion Vision Systems. The request is for the most current technology to accommodate the growing demand of the medical center to provide safe standard of care to our veteran population. Alcon Surgical has been dedicated to providing best in class service and equipment throughout our relationship. a. Identification of Possible Follow-on Work. N/A b. Identification of Potential Conflicts of Interest (COI). N/A c. Identification of Non-Disclosure Requirements. N/A d. Packaging, Packing and Shipping Instructions.. Write the PO number associated with the sale on every box to be shipped. Send out all procured devices in a single shipment. Devices should be shipped to: NMVAHCS, Warehouse 1501 San Pedro SE Albuquerque NM 87109 Warehouse is to deliver the devices to: Logistics for equipment data base sticker placement, Attn: Rhonda Lyons A work order is created for Biomedical Engineering inspection to be done at the warehouse. Once inspection is completed, deliver to building 41 3b-107 Attn: Marcela/Nate e. Inspection and Acceptance Criteria. All items should be free of defects. Biomedical engineering staff will judge the quality of the devices procured within the 1 year warranty. If the products are not up to standards, then a repair, exchange, or credit on behalf of the vendor will be required. 10. Risk Control: N/A All items should be free of defects and are covered by manufacturer warranty.. 11. Place of Performance. The Raymond G. Murphy VA Health Care Center Operating Room 12.
