11/17/201610:01 AM VETERANS AFFAIRS MEDICAL CENTER SCOPE OF WORK REPAIR OF MEDRAD INJECTOR (FOR RADIOLOGY) SECTION A Contractor shall furnish all labor, materials, parts, equipment, tools to repair MEDRAD Spectris Solaris injector used in radiology located at Michael E DeBakey VA Medical Center, 2002 Holcombe Blvd. Houston TX 77024. All repairs must be performed by manufacturer licensed professionals using only original equipment manufacturer parts. Following the repair, vendor must demonstrate to a MEDVAMC Biomed Technician that the equipment runs properly according to manufacturer specified levels. The contract price shall be firm-fixed, all inclusive not limited to specified products/equipment, installation, and setup/configuration. All companies must provide documentation proving that their technicians are certified from the original manufacturer of the equipment to provide service to the equipment specified above. Additionally, the contractor must provide documentation of at a minimum 3 occurrences of similar work. All parts must be certified new from the original manufacturer. DESCRIPTION OF WORK Contractor will provide the repairs needed for the MEDRAD injector (S/N 45238) which must have immediate emergency repairs completed. Upon contract award, the company will have 72 hours to ship the parts to the VA hospital (location above) with the attention to Morris McKenzie located in BA-200. They will then have an additional 48 hours for completing the repair. Contractor will be contacted to verify and document that the MEDRAD injector is working properly and communicate with MEDVAMC staff when the unit is ready for use. Contractor will coordinate with MEDVAMC staff to ensure no impact to patient care during maintenance. PRIVACY/SECURI TY The contractor will not have electronic connectivity to the devices and will not have access to patient databases or receive screenshots with PHI present. Any electronic or magnetic data storage devices such as hard drives, etc. shall be turned into the ISO. Electronic or magnetic data storage devices are not to be removed from VA property. 1 Special Notice This is a Sources Sought Notice to Award a Firm Fixed Contract in accordance with FAR 6.203. The Department of Veterans Affairs at 2002 Holcombe BLVD Houston,TX. 77030-4298,has a requirement for Medrad Injector repair parts. This notice is published to conduct market research to determine if there are available distributors of the products/services. Interested parties responding to this sources sought request shall submit the following information at a minimum: (1) company name and address, point of contact with phone number, and DUNS number; (2) documentation relating to capability of performance; (3) any other pertinent company documentation. This is not a request for proposal any notice received must show clear and compelling evidence that competition would be advantageous to the Government please see statement of work attached. A determination to compete this proposed action is solely within the discretion of the Government.
